摘　　要：目的探讨骨化三醇联合缬沙坦治疗早期糖尿病肾病(DN)对肾功能的影响。方法将72例早期DN患者随机分为对照组和观察组,每组36例。在糖尿病综合治疗的基础上,对照组给予缬沙坦治疗,观察组给予骨化三醇联合缬沙坦治疗,疗程均为6个月。比较治疗前后两组患者24 h尿白蛋白(24h UA)定量、超敏C反应蛋白(hs-CRP)、血压、血清肌酐(SCr)、甲状旁腺素(PTH)水平。结果治疗后,两组患者24h UA、hs-CRP、SCr、收缩压、舒张压均较治疗前降低,血钙较治疗前升高,且观察组患者24h UA、hs-CRP、SCr水平低于对照组,血钙高于对照组(均P<0.05)。结论与单用缬沙坦比较,骨化三醇联合缬沙坦治疗早期DN患者能更好地降低尿白蛋白水平,保护肾功能,延缓肾损伤。Objective To investigate the effect of ealeitriol combined with valsartan on renal function in the patients with early diabetic nephropathy（DN）. Methods A total of 72 patients with early DN were randomly divided into control group and observation group,with 36 cases in each group. On the basis of comprehensive treatment of diabetes, the control group was given valsartan, while the observation group received calcitriol combined with valsartan. Both groups were treated for 6 months. The levels of 24 h urinary albumin （24hUA） ,high-sensitiviU C-reactive protein（ hs-CRP）, blood pressure, serum creatinine （SCr） and parathyroid hormone （PTH） were compared between the two groups before and after treatment. Results After treatment, the levels of 24hUA, hs-CRP, SCr, SBP and DBP decreased but the level of serum calcium increased in the two groups compared to those before treatment, and the levels of 24hUA, hs-CRP and SCr were lower but the level of serum calcium was higher in the observation group compared to the controls（ all P 〈 0.05 ）. Conclusion Compared to the monotherapy of valsartan, ealeitriol combined with valsartan can better reduce the level of urinary albumin, protect renal function and delay renal impairment in the patients with early DN.
