早期无创正压通气联合药物治疗在急性左心衰竭患者中的应用价值

摘　　要：目的:分析早期无创正压通气(NPPV)联合药物治疗在急性左心衰竭患者中的应用价值。方法:选取在我院就诊的80例急性左心衰竭患者,按随机数字表法分为两组,每组40例。对照组采用常规对症支持治疗,观察组在此基础上联合早期无创正压通气治疗,比较两组患者治疗前后呼吸(R)、心率(HR)、氧/二氧化碳分压(PaO_2/Pa CO2)、血氧饱和度(SaO_2)和血浆钠尿肽(BNP)水平及治疗效果。结果:观察组治疗24 h后R、HR、PaO_2、PaCO_2、SaO_2、BNP水平均优于对照组,差异有统计学意义(P<0.05);对照组治疗总有效率为65.0%,低于观察组的92.5%,差异有统计学意义(P<0.05)。结论:无创正压通气联合药物治疗急性左心衰竭利于改善患者血气分析指标,减轻心力衰竭症状,较常规治疗效果更佳。Objective： To analyze the application value of early non-invasive positive pressure ventilation（NPPV） combined with drugs in the treatment of patients with acute left heart failure（ALHF）. Methods：80 ALHF patients in our hospital were divided into two groups by random number table,with 40 cases in each group. Patients in the control group were given routine symptomatic and supportive treatment,while those in the observation group were treated with early NPPV based on the treatment received by the control group. The respiratory rate（R）,heart rate（HR）,arterial partial pressure of oxygen/partial pressure of carbon dioxide（PaO2/Pa CO2）,oxygen saturation（SaO2）,plasma level of brain natriuretic peptide（BNP） of patients between both groups before and after treatment,and the curative effects between both groups were compared. Results： 24 h After treatment,R,HR,PaO2,PaCO2,SaO2,BNP in the obervation group were better than those of the control group,which were of statistical differences（P〈 0.05）; the total effective rate of control group was 65.0%,which was lower than 92.5% of observation group,which was of statistical difference（P 〈0. 05）. Conclusion： NPPV combined with drugs in the treatment of ALHF can improve blood gas indexes and reduce symptoms of heart failure,whose curative effects are better than those of the routine treatment.
