阵发性房颤射频消融术后空白期内房性心律失常与房颤远期复发的关系

摘　　要：目的:探讨阵发性房颤射频消融术后空白期内房性心律失常与房颤远期复发的关系。方法:将在我院及其他医院收治的60例首次进行环肺静脉电隔离射频消融术的阵发性房颤患者作为研究对象,按照随访结果将患者分为房颤复发组以及无复发组,比较两组患者术后3个月内房性心律失常发生率。结果:在对60例进行随访的过程中,有41例没有复发,19例复发,两组患者在基本资料上比较差异无统计学意义,而在左心房内径上差异有统计学意义(P<0.01)。两组患者术后1个月、2个月、3个月、1年心电图诊断房性心律失常发生率差异有统计学意义,复发组心律失常发生率明显高于无复发组,组间数据差异有统计学意义(P<0.05)。结论:阵发性房颤射频消融术后早期复发虽然不能直接表示消融失败或是晚期复发,可是早期房性心律失常发作较为频繁的话,患者晚期房颤复发的可能性也就会明显提升。Objective： To investigate the relationship between atrial arrhythmia and the long-term recurrence of atrial fibrillation after radiofrequency ablation of paroxysmal atrial fibrillation. Methods： 60 patients with paroxysmal atrial fibrillation treated by circumferential pulmonary vein isolation（CPVI）radiofrequency ablation for the first time in our hospital were selected as the subjects,who were divided into the recurrence group and non-recurrence group of atrial fibrillation according to the follow-up results.The occurrence rate of atrial arrhythmia in both groups 3 months after operation was compared. Results： In the process of follow-up for the 60 patients,41 patients were found with no recurrence,19 patients relapsed,and there were no statistical differences of the baic information and the left atrium diameter between both groups（P〈 0. 01）. There were statistical differences of the incidence of electrocardiogram diagnosis of ventricular arrhythmia between both groups 1 month,2 months,3 months and 1 year after operation; incidence of the atrial arrhythmia of the recurrence group was higher than that of the nonrecurrence group,and there were statistical differences between groups（P〈 0. 05）. Conclusion： After radiofrequency ablation of paroxysmal atrial fibrillation,the early recurrence of paroxysmal atrial fibrillation may not directly show it is ablation failure or late recurrence,but early atrial arrhythmia attacks more frequent,the possibility of late recurrence of atrial fibrillation in patients will be obviously increased.
