摘 要:子宫内膜异位症(Endometriosis,EMs)是一种妇科常见病、疑难病,可致继发性痛经、性交痛、慢性盆腔痛、不孕等症状。子宫内膜异位症血管生成机制是疾病发生发展的主要机制之一,其相关性疼痛的机制仍不明确,可能与神经纤维的异常、神经生长因子及其受体的异常表达、炎性因子的异常表达等有关。近期相关研究提示,血管生成与新生神经纤维的生长存在内在内在联系,即促神经血管生成,并共同影响内异症疼痛的发生。促神经血管形成可能是EMs主要机制之一。治疗方面,除了常规的手术及药物治疗外,中西医亦进行丰富的探索,抗血管生成药物为目前研究的热门方向之一。现代研究提示活血化瘀中药具有抗血管生成作用。促神经血管生成机制亦为临床治疗子宫内膜异位症相关性疼痛提供新思路,对子宫内膜异位症相关性疼痛与血管生成进行中西医研究进行综述。Endometriosis is a common difficult disease. It is often associated with secondary dysmenorrhea,dyspareunia,pelvic pain and infertility. Endometriosis is an angiogenesis-dependent disorder. The mechanism of endometriosis-associated pain still remains unclear. Endometriosis-associated pain symptoms may be related to the abnormal expression of nerve fibers,nerve growth factor and its receptors,abnormal inflammatory factors. The relevant researchs suggests that angiogenesis is linked to the growth of new nerve fibers,which is called neuroangiogenesis. Both of these affect the occurrence of the pain. In addition to routine surgery and drug therapies,Chinese and Western medicine are also explored. Anti-angiogenic therapies have become a hotspot in endometriosis treatment. Some researches suggest that blood-activating and stasis-removing drugs can significantly inhibit the development,growth and angiogenesis of experimental endometriosis. There is an effective clinical effect on pain treatment in other areas. We also consider the therapeutic implications of neuroangiogenesis and propose potential treatments that may become useful in the control endometriosis pain. In this review,we focus on the researches on the endometriosis-associated pain and neuroangiogenesis mechanism.
