温肾泄浊汤联合透析治疗慢性肾功能衰竭临床疗效

摘　　要：目的：探讨温肾泄浊汤联合血液透析治疗慢性肾功能衰竭的临床疗效。方法：60例慢性肾功能衰竭患者,随机分为对照组（30例）和观察组（30例）,对照组给予透析治疗,观察组给予温肾泄浊汤联合透析治疗。治疗8周后,观察两组患者的临床疗效,气短懒言、腰膝酸软、倦怠乏力、食少纳呆等中医症候积分,比较C反应蛋白（CRP）、肿瘤坏死因子-α（TNF-α）和白细胞介素-6（IL-6）等炎症因子水平,内生肌酐清除率（CCr）、血肌酐（SCr）、血红蛋白（Hb）和血尿素氮（BUN）等肾功能指标。结果：治疗8周后,观察组93.33%显著高于对照组70.00%（χ2=5.454,P=0.020）。观察组气短懒言、腰膝酸软、倦怠乏力、食少纳呆等积分显著低于对照组（P〈0.05）。观察组炎症因子CRP、TNF-α和IL-6水平显著低于对照组（P〈0.05）。观察组CCr和Hb显著高于对照组,而SCr和BUN显著低于对照组（P〈0.05）。结论：温肾泄浊汤联合透析治疗慢性肾功能衰竭可改善患者中医症状,降低炎症反应,提高肾功能,具有较好的临床疗效。Objective： To study the clinical efficacy research of Wenshen Xiezhuo Decoction combined with dialysis in treatment of patients with chronic renal failure. Methods： Sixty patients with chronic renal failure were randomly divided into the control group（ 30 cases） and the observation group（ 30 cases）. The control group was given dialysis treatment,and the observation group was given Wenshen Xiezhuo Decoction combined with dialysis therapy. After 8 weeks treatment,clinical efficacy,shortness of breath,lumbar debility,languid lazy words,appetite and stay time in two groups were observed. The levels of inflammation factors including C-reactive protein（ CRP）,tumor necrosis factor alpha（ TNF-α） and interleukin 6（ IL-6）,renal function indexes including endogenous creatinine clearance rate（ CCr）,serum creatinine SCr）,hemoglobin（ Hb） and blood urea nitrogen（ BUN） were compared. Results： After 8 weeks treatment,the total effective rate 93. 33% in the observation group was significantly higher than the control group＇s 70. 00%（ χ2=5. 454,P = 0. 020）. The scores of shortness of breath,lumbar debility,languid lazy words,appetite and stay time in the observation group were significantly lower than those of the control group（ P 〈0. 05）. The levels of inflammatory factors including CRP,TNF-α and IL-6 in the observation group were significantly lower than those of the control group（ P 〈0. 05）. The levels of CCr and Hb in the observation group were significantly higher than those of the control group,and the SCr and BUN in the observation group were significantly lower than those of the control group（ P 〈0. 05）. Conclusion： Wenshen Xiezhuo Decoction combined with dialysis in treatment of patients with chronic renal failure can improve TCM symptoms,reduce inflammation,improve renal function and has better clinical curative effect.
