摘  要:目的探讨双侧岩下窦静脉采血(bilateral inferior petrosal sinus sampling,BIPSS)对促肾上腺皮质激素(adrenocorticotropic hormone,ACTH)依赖性库欣综合征的诊断价值。方法回顾性分析9例疑似ACTH依赖性库欣综合征病例资料,行BIPSS和垂体MRI检查,结合术后病理比较BIPSS及垂体MRI诊断准确率。结果 9例病人中8例岩下窦静脉与外周静脉血ACTH比值>2,诊断库欣病8例,并经手术证实,诊断符合率100%;垂体MRI定性诊断库欣病6例,诊断符合率为75%。1例病人BIPSS结果考虑为异位来源,并经手术证实。结论 BIPSS是诊断ACTH依赖性库欣综合征的较好方法。Objective To investigate the diagnosis value of bilateral inferior petrosal sinus sampling(BIPSS) in adrenocorticotropic hormone(ACTH)-dependent Cushing's syndrome. Methods Clinical data of 9 patients with suspected ACTH-dependent Cushing's syndrome were analyzed retrospectively. The BIPSS and pituitary MRI examinations were performed on the patients and the results of the two examinations were compared according to the pathological results. Results The ratio more than 2 between inferior petrosal sinus venous ACTH and peripheral venous ACTH was found in 8 patients, who were diagnosed as having Cushing's disease and confirmed by surgery. The diagnosis coincidence rate was 100%. However, 6 patients were diagnosed with Cushing's disease by pituitary MRI and the diagnosis coincidence rate was 75%. One patient was diagnosed with ectopic origin and confirmed by surgery.Conclusion BIPSS is useful for diagnosis of ACTH-dependent Cushing's syndrome.
