摘 要:目的探讨高分辨率锥形束CT(CBCT)对人工耳蜗植入(CI)术后耳蜗内电极的评估价值。方法选取因双耳极重度感音神经性耳聋行CI术患者16例,术后行高分辨率CBCT检查和64排螺旋CT植入侧扫描,比较两种检查在测定耳蜗内电极的位置、计数电极数、测量电极植入深度等方面的差异。结果 8例患者植入COCHLEAR公司22个电极人工耳蜗(Nucleus CI512),8例患者植入NUROTRON公司24个电极人工耳蜗(CS-10A)。两种检查对CI术后耳蜗内电极计数的差异有统计学意义(P<0.05),高分辨率CBCT优于64排螺旋CT。两种检查对CI术后耳蜗内电极位置判断的差异无统计学意义(P>0.05)。多排螺旋CT直接法测得CI术后耳蜗内电极植入深度>CBCT直接法>CBCT间接法,CBCT间接法测量电极植入深度更加接近于术中实际测量值。结论高分辨率CBCT不但能显示耳蜗内电极位置,并能清晰显示及测量耳蜗内电极植入深度、数目等指标,在CI术后可以代替多排螺旋CT进行耳蜗内电极的影像学评估。Objective To investigate the value of cone beam CT(CBCT)in assessment of cochlear implant electrodes after cochlear implantation.Methods Sixteen patients with severe binaural sensorineural hearing loss after CI were enrolled.High resolution CBCT scan and 64slice CT scan were performed to assess the position,the number and the depth of electrode implantation.Results Eight patients were implanted with COCHLEAR 22-electrode cochlear implant(Nucleus CI512),and 8 patients with NUROTRON 24-electrode cochlear implant(CS-10A).There was significant difference in counting the number of cochlear electrodes between the two examinations(P〈0.05),and the high resolution CBCT was superior to 64 slice CT.There was no significant difference between the two examinations in judging the position of electrode after CI(P〈0.05).The depth of electrodes measured with direct method of multi-detector CT〉that of direct method of CBCT〉that of indirect method of CBCT.The depth of electrodes measured with indirect method of CBCT was closer to the actually measured value.Conclusion High resolution CBCT can clearly display the electrode position,and measure the length and number of electrodes for cochlear implant.
