以心功能分级为依据的中医分型治疗慢性心力衰竭临床观察

摘　　要：目的:观察以心功能分级为依据的中医分型治疗慢性心力衰竭(chronic congestive heart failure,CHF)的临床疗效。方法:将CHF患者60例随机分为治疗组和对照组各30例,对照组单纯西医治疗,治疗组在对照组治疗的基础上以心功能分级为辨证论治基础治疗,治疗4周为1个疗程,1个疗程后观察总有效率,血浆氨基末端脑钠肽前体(NT-proBNP)浓度与左室射血分数(Left ventricular ejection fraction,LVEF)。结果:总有效率治疗组为96. 7%,对照组为83. 3%;2组比较差异有统计学意义(P<0. 05);2组LVEF治疗后升高(P<0. 05),治疗组升高更明显;NT-proBNP浓度2组治疗后下降,治疗组下降更明显(P<0. 05),结论:以心功能分级为依据的中医辨证分型治疗CHF疗效优于单纯西医治疗。Objective： To observe clinical effects of TCM pattern therapy based on cardiac functional gradingin treating chronic congestive heart failure （CHF）. Methods： Sixty CHF patients were randomized into the treatmentgroup and the control group, 30 cases each group, the control group received the therapy of western medicine, andthe treatment group basic treatment of syndrome differentiation and treatment based on cardiac functional grading,four weeks one course, left ventricular ejection fraction （LVEF）, NT-proBNP and total effective rate were observedafter one course. Results： Total effective rate of the treatment group was 96.7%, higher than 83.3% of the controlgroup, the difference had statistical meaning in the comparisons of total effective rates between both groups（P〈0.05）;LVEF of both groups raised after treating （P〈0.05）, the raise of the treatment group was more obvious; theconcentrations of NT-proBNP lowered in both groups after treating, and the decrease of the treatment group wasmore obvious（P〈0.05）. Conclusion： Clinical effects of syndrome differentiation and patterns based on cardiacfunctional grading are superior to only western medicine in treating CHF.
