膝关节镜联合去神经化术治疗膝骨性关节炎的疗效

摘　　要：目的探讨膝关节镜下关节清理术联合去神经化治疗膝骨性关节炎(KOA)的疗效。方法将88例KOA患者按照随机数表法分为两组,各44例。对照组采用膝关节镜下清理术治疗,观察组采用膝关节镜下清理术联合去神经化术治疗。术后对两组患者的疼痛程度、膝关节功能及生活质量进行评估。结果患者均获得随访,时间6～12个月。术后1、3、6个月,疼痛VAS评分观察组明显低于对照组(P <0. 05),关节活动度观察组明显优于对照组(P <0. 05)。末次随访时,膝关节功能优良率、生活质量AIMS2-SF评分观察组均高于对照组(P <0. 05);并发症发生率观察组低于对照组(P <0. 05)。结论膝关节镜下清理术联合去神经化治疗可有效缓解KOA患者疼痛症状并提高生活质量。Objective To investigate the effect of knee arthroscopy combined with denervation in the treatment of primary knee osteoarthritis （KOA）. Methods The 88 cases of KOA patients were according to random number table method, divided into two groups, 44 cases in each group.The control group was treated with knee arthroscopic debridement. The observation group was treated with knee arthroscopic debridement combined with denervation.After the operation, the pain degree, knee function, and quality of life of the two groups were evaluated. Results Patients were followed up for 6~12 months.The pain VAS at 1, 3, and 6 months after operation were significantly lower than those of the control group（ P 〈0.05）. The observation group of joint mobility was significantly better than the control group（ P 〈0.05）.At the last follow-up, the knee function, quality of life score, and AIMS2-SF score were higher in the observation group than in the control group （ P 〈0.05）. The incidence rate of complications in the observation group was lower than that in the control group （ P 〈0.05）. Conclusions Arthroscopic debridement combined with denervation therapy can effectively relieve pain symptoms in KOA patients and improve the patients′ quality of life.
