摘　　要：目的总结关节镜辅助下Orthocord线固定前交叉韧带(ACL)胫骨髁间棘撕脱性骨折的疗效。方法对35例ACL胫骨髁间棘撕脱性骨折患者采取关节镜辅助下Orthocord线固定治疗。术后6周行膝关节X线检查,评估骨折愈合情况;术后3、6、12个月进行Lysholm膝关节评分、前抽屉试验和Lachman试验,评估患者术后膝关节生理功能、ACL恢复情况。结果患者均获得随访,时间12～24个月。术后6周膝关节X线、CT检查显示骨折复位且愈合,状态良好。术后3、6、12个月,前抽屉试验和Lachman试验均为阴性,Lysholm膝关节评分分别为(84. 2±2. 7)分、(89. 6±3. 8)分、(95. 8±4. 1)分,膝关节活动度为0°～130°,膝关节生理功能基本恢复至正常水平,未出现膝关节不稳及关节绞锁、关节畸形等并发症。结论关节镜辅助下Orthocord线固定ACL胫骨髁间棘撕脱性骨折疗效显著,是一种较好的方法。Objective To summarize the curative effect of arthroscopic and Orthocord suture fixation for treating anterior cruciate ligament（ACL） and tibia avulsion fracture of tibial eminence. Methods The 35 pantients with ACL and tibia avulsion fracture of tibial eminence were treated with arthroscopic and Orthocord suture fixation. All patients were inspected by tibial position X-ray at 6 weeks after surgery, underwent the assessment of fracture healing condition.Lysholm knee score, anterior drawer test and Lachman test were evaluated at 3, 6, 12 months after surgery, postoperative knee function recovery,ACL recovery were evaluated. Results All patients were followed up from 12 months to 24 months.The X-ray examination showed fracture reduction and healing had a good condition at 6 weeks after surgery. The anterior drawer test and Lachman test were negative at 3, 6, 12 months after surgery, and Lysholm knee score was 84.2±2.7, 89.6±3.8 and 95.8±4.1, respectively. The activity degree of knee was 0°～130°, and the knee joint function recovered to normal level, without joint instability symptoms, joint interlocking, joint deformity and other complications. Conclusions Arthroscopic and Orthocord suture fixation for treating ACL and tibia avulsion fracture of tibial eminence has excellent effect, less postoperative complications, and it is a better way.
