原发性和继发性良性阵发性位置性眩晕临床特征分析

摘　　要：目的对比分析原发性和继发性良性阵发性位置性眩晕(benign paroxysmal positional vertigo, BPPV)患者的临床特征。方法回顾性分析2017年1~6月在武警后勤学院附属医院耳鼻咽喉科诊治的392例BPPV患者的临床资料,比较原发性和继发性BPPV患者在年龄、性别分布及治疗有效率、成功复位治疗后残留头晕症状发生率、复发率方面的差异。结果原发性与继发性BPPV分别为362例(92.3%)和30例(7.7%),2组患者的性别和年龄差异无统计学意义(P> 0.05);首次复位治疗后的有效率分别82.8%和26.7%(P <0.05);治疗后残留头晕症状发生率分别为7.2%和20.0%(P <0.05);6个月内复发率分别为10.2%和28.4%(P <0.05)。结论继发性BPPV所占比例虽较小,但复位治疗有效率低,复位治疗后残余头晕症状发生率和复发率均较高,临床工作中应重视对继发性BPPV的及时干预治疗。Objective To analyze the clinical features of patients with primary and secondary benign paroxysmal positional vertigo （BPPV）. Methods A total of 392 cases, including 360 cases （92.3%） with primary BPPV and 30 cases （7.7%） with secondary BPPV, was analyzed retrospectively. The differences in age, gender, effective rate of repositioning procedures, incidence of residual dizziness and recurrence rate were evaluated between two groups. Results There were not no significant differences in age and gender between patients with primary and secondary BPPV （P 〉 0.05）. Compared with primary BPPV, secondary BPPV showed a lower effective rate after single repositioning maneuver （26.7% vs. 82.8%, P 〈 0.05）, while a higher incidence of residual dizziness （20.0% vs. 7.2%, P 〈 0.05） and a higher recurrent rate （28.4% vs. 10.2%, P 〈 0.05）. Conclusion Although the proportion of secondary BPPV in all BPPV cases is small, but the efficiency of repositioning treatment is low, the incidence of residual dizziness after successful repositioning and recurrence rate are high. More attention should be paid to secondary BPPV, with early intervention and treatment.
