摘 要:目的探讨茵陈蒿汤预防α-萘异硫氰酸酯(ANIT)诱导的大鼠急性肝内胆汁淤积的作用,观察其对肝内胆汁淤积大鼠肝功能指标及肝脏病理学影响。方法 54只大鼠随机分为正常组、模型组、中药组各18只,各组根据不同处理时间再分为24 h、48 h、72 h三个亚组,每个亚组6只。模型组及中药组大鼠一次性给予2%ANIT按100 mg/(kg·d)灌胃造模,正常组仅给予同等容积色拉油灌胃。造模2 h后中药组大鼠给予茵陈蒿汤中药煎剂1 ml/100 g灌胃,每日1次;模型组和正常组给予等量生理盐水灌胃,每日1次。造模成功后分别于24 h、48 h、72 h取相应时相动脉血检测丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、总胆红素(TBil)、直接胆红素(DBil)、γ-谷氨酰转移酶(GGT)、碱性磷酸酶(ALP)、总胆汁酸(TBA)等,并取肝右叶同一部位组织进行HE染色。结果模型组各时相血清ALT、AST、TBIL、DBIL、ALP、GGT、TBA较正常组明显升高,肝细胞、胆管细胞肿胀坏死和炎细胞浸润较正常组严重;中药组各时相血清ALT、AST、TBIL、DBIL、ALP、GGT、TBA较模型组明显改善,肝细胞、胆管细胞肿胀坏死、炎细胞浸润等情况明显减轻。结论茵陈蒿汤可明显降低ANIT灌胃诱导的急性肝内胆汁淤积大鼠血清生化指标水平,改善肝组织病理改变。Objective To investigate how Yinchenhao Decoction prevented medicine-induced acute intrahepatic cholestasis and observe liver function and pathology changes. Methods 54 rats were randomized into normal group, model group and Chinese medicine group (n=18 rats respectively). Each group was divided into subgroups of 24 h, 48 h and 72 h treatment (n=6 rats respectively). The rats in model group and Chinese medicine group were given 2% alpha-naphthyl-iso-thiocyanate (ANIT) 100 mg/(kg·d) gavage once a day, while normal group same volume of salad oil. After 2 h, the Chinese medicine group were given Yinchenhao Decoction 1 ml/100 g once a day while model group and normal group same amount of normal saline. The arterial blood samples of 24 h, 48 h and 72 h were to be analyzed regarding alanine aminotransferase (ALT), glutamine transaminase (AST), total bilirubin (TBIL), direct bilirubin (DBIL), gamma glutamyl transferase (GGT), alkaline phosphatase (ALP) and total bile acid (TBA). The specific part of right lobe liver were biopsied. Results The model group saw higher levels of ALT, AST, TBIL, DBIL, ALP, GGT, TBA in each phase and more liver tissue damage than normal group. The Chinese medicine group saw relieved ALT, AST, TBIL, DBIL, ALP, GGT, TBA and improved liver pathology than model. Conclusion Yinchenhao Decoction can significantly relieve ANIT-induced acute intrahepatic cholestasis in rats biochemically as well as pathologically.
