摘 要:目的研究桥本甲状腺炎患者外周血调节性T细胞(Treg)与Th17细胞的动态变化与意义。方法采用回顾性研究方法,选取2016年8月至2017年4月陕西省安康市人民医院内分泌科收治的桥本甲状腺炎患者40例(观察组)以及同期健康体检者40例(对照组),比较两组外周血单个核细胞(PBMC)中Th7/Treg细胞比值、细胞特异性转录因子Foxp3、RORγt的mRNA表达以及血清中的IL-17A与TGF-β含量。结果观察组患者的TSH、TPO-Ab、Tg Ab明显高于对照组,FT3、FT4明显低于对照组(P<0.05);观察组PBMC中的Treg与Th17细胞比率明显低于对照组(P<0.05),Th7/Treg细胞比值明显高于对照组(P<0.05),Foxp3中mRNA表达水平明显低于对照组(P<0.05),RORγt的mRNA表达水平明显高于对照组(P<0.05);观察组患者血清IL-17A水平明显高于对照组(P<0.05)。结论桥本甲状腺炎患者的Th17/Treg细胞比值会发生明显的改变,可能与甲状腺抗体存在联系,Th17/Treg细胞亚群免疫功能失衡与疾病有密切的关系。Objective To study the changes and significance of peripheral blood regulatory T cells (Treg) and Th17 cells) in patients with Hashimoto's thyroiditis. Methods The clinical data of 40 patients with Hashimoto's thyroiditis admitted during August 2016 to April 2017 were retrospectively reviewed and analyzed. These patients were listed as observation group and 40 cases of healthy persons were selected as control group. The detection of peripheral blood mononuclear cells (PBMC) in Th7/ cell ratio, Treg cell specific transcription factor Foxp3, ROR gamma t expression of mRNA and IL-17A in serum and the content of TGF- beta had detected and compared. Results The serum levels of TSH, TPO-Ab and TgAb in patients of observation group were significantly higher than those of control group, the blood levels of FT 3 and FT 4 were significantly lower than those of control group ( P 〈0.05). ObTreg in PBMC and Th17 cell ratio in observation group were significantly lower than those of control group ( P 〈0.05). The Th7/Treg cell ratio was significantly higher than that of control group ( P 〈0.05). The expression level of Foxp3 mRNA was significantly lower than that of control group ( P 〈0.05). The expression level of ROR gamma t mRNA was significantly higher than that of control group ( P 〈0.05). Serum level of IL-17A in observation group was significantly higher than that of control group ( P 〈0.05). Conclusion The Th17/Treg cell axis in patients with Hashimoto's thyroiditis can significantly be changed and it may be associated with thyroid antibody, and the imbalance of immune function of Th17/Treg cells is closely related to this disease.
