摘  要:目的观察自制弹力髋兜加压包扎人工全髋关节手术伤口,对伤口引流量、并发症等的影响。方法初次行人工全髋关节置换术患者62例,随机分为弹力髋兜加压包扎组31例,常规辅料包扎组31例,统计术后48 h两组伤口引流量、血红蛋白量、输血量,记录术前术后大小腿周径,了解肢体肿胀情况。结果弹力髋兜包扎组伤口引流量(262±57)mL,明显少于普通辅料包扎组引流量(386±78)mL(t=7.147,P<0.05);弹力髋兜包扎组输血量(58±54)mL(输浓缩红细胞折算),明显少于普通辅料包扎组输血量(164±82)mL输浓缩红细胞折算)(t=7.146,P<0.05);弹力髋兜包扎组大小腿周径与普通辅料包扎组大小腿周径比较差异无统计学意义。结论弹力髋兜包扎加压包扎人工全髋关节手术伤口,可明显减人工全髋关节术后伤口出血,减少输血量,同时不影响患肢血液循环。Objective To observe the effects of self-made elastic hip joint surgical wound on wound drainage and complications. Methods 62 patients with total hip arthroplasty for the first time, Thirty-one cases were randomly divided into elastic hip pocket pressure dressing group and 31 cases of conventional auxiliary dressing group. In the 48 hours after the operation, two groups of wound drainage, hemoglobin and blood transfusion were recorded, and the diameter of the crus was recorded before and after surgery to understand the swelling of the limbs. Results The wound drainage rate(262±57) mL of the elastic hip pocket was significantly less than that of the normal auxiliary binder(386±78) mL(t=7.147, P0.05); The amount of blood transfusion(58±54) was significantly less than that of the conventional auxiliary binder(164±82) mL(t=7.146, P0.05). There's no statistically significant difference between the elastic and the lower leg and the lower leg weeks and the lower leg weeks. Conclusion Elastic hip pocket dressing and pressure dressing for total hip joint surgical wound can obviously reduce the bleeding after total hip joint operation, reduce blood transfusion volume, without the risk of lower limb vein thrombosis.
