精准肝切除治疗区域型肝胆管结石的临床疗效

摘　　要：目的:探讨精准肝切除治疗区域型肝胆管结石的临床疗效。方法:收治区域型肝胆管结石患者86例,分为两组,每组各43例。对照组给予非规则性肝切除治疗,试验组给予精准肝切除治疗,比较两组临床疗效。结果:试验组手术时间、术中出血量、术后住院时间少于对照组,差异有统计学意义(P<0.05)。两组肝断面感染率、残余结石发生率、结石复发率、胆漏发生率比较,差异有统计学意义(P<0.05)。两组肝功能恢复时间比较,差异无统计学意义(P>0.05)。结论:精准肝切除治疗区域型肝胆管结石的临床疗效显著,能够降低术后并发症以及缩短术后住院时间。Objective：To investigate the clinical effect of precise hepatectomy in the treatment of regional hepatolithiasis.Methods： 86 cases of patients with regional hepatolithiasis were divided into two groups.The control group was treated with irregular hepatectomy,and the experiment group was treated with precise hepatectomy,the clinical effectbetween the two groups was cornpared.Resuhs：The operation time,intraoperative blood loss and postoperative hospitalization time in the experiment group were less than those in the control group,the difference was statistically significant （P 〈 0.05）.There was significant difference of the incidence of liver section infection,the incidence of residual stones,the recurrence rate of calculus and the incidence of bile leakagebetween the two groups （P 〈 0.05）.There was no significant difference in the recovery time of liver function between the two groups （P〉 0.05）.Conclusion：Precise hepatectomy was effective in the treatment of regional hepatolithiasis,which could reduce the postoperative complications and shorten postoperative hospitalization time.
