PRECEDE护理模式对老年慢性乙型肝炎患者的临床效果及生活质量的影响

摘　　要：目的探讨在慢性乙型肝炎老年患者中应用PRECEDE(Predisposing,Reinforcing,and Enabling Causesin Educational Diagnosos and Evaluation,PRECEDE)护理模式的临床效果。方法选取2015年6月至2016年6月期间淮安市第四人民医院接收的82例慢性乙型肝炎老年患者,按随机数字表法分为对照组(n=41,实施常规护理干预)与观察组(n=41,实施PRECEDE护理模式),观察两组患者干预后的临床效果、遵医行为及生活质量情况。结果观察组患者的临床有效率及满意度高于对照组(P<0. 05);并且,观察组患者合理饮食、遵医嘱用药、适当运动、定期复查、戒烟戒酒率均高于对照组(均P<0. 05);此外,护理干预后,观察组心理功能、躯体功能、社会功能评分及总分均较干预前明显改善(P<0. 05),且优于对照组(P<0. 05)。结论在慢性乙型肝炎老年患者中开展PRECEDE护理模式,能够提高临床疗效,改善患者遵医行为和生活质量,提高患者满意度,具有推广价值。Objective To explore the clinical effect of Predisposing, Reinforcing, and Enabling Causesin Educational Diagnosos and Evaluation（PRECEDE） nursing mode on the elderly patients with chronic hepatitis B.Methods Eighty-two cases of elderly patients with chronic hepatitis B treated by the Fourth People s Hospital of Huaian from June 2015 to June 2016 were selected and divided into control group（ n =41, carried on the routine nursing intervention） and observation group（ n =41, with PRECEDE model nursing） randomly. The clinical effect, compliance and life quality of the two groups after the intervention were compared.Results Compared with the control group, the clinical efficiency and satisfaction of the patients in the observation group were significantly higher （ P 〈0.05）.At the same time, compared with the control group,the patients in the observation group had a better performance in the diet, medication compliance, regular exercise, regular review, smoking cessation and alcohol abstinence rate （ P 〈0.05）. In addition, the psychological function, body function, social function score and total score of the observation group improved significantly （ P 〈0.05）.Conclusions The application of PRECEDE nursing mode can improve the clinical effect and compliance behaviors for elderly patients with chronic hepatitis B, which is worthy of popularization.
