摘  要:目的通过回顾性分析飞行人员体检资料,了解和分析高甘油三酯血症在飞行人员中的流行情况及危险因素,为疗养院开展健康管理提供依据。方法收集2013-2017年参加大体检的飞行人员体检资料,采用统计学方法分析高甘油三酯血症在飞行人员中的流行情况及危险因素。结果 (1)2013-2017年飞行人员高甘油三酯血症患病趋势增加(P<0.001),5年平均患病率为17.2%,2017年飞行人员高甘油三酯血症患病率是2013年的2.1倍。(2)病例组飞行人员平均年龄(36.6±7.6)岁、平均飞行时间(2455±1215)h,均明显高于对照组(P<0.001)。(3)Spearman相关分析结果表明,飞行人员患高甘油三酯血症风险与年龄呈正相关(相关系数r=0.361,P<0.001)、与飞行时间呈正相关(r=0.377,P<0.001)、与婚姻状况(未婚)呈负相关(r=-0.128,P<0.001)。(4)多因素Logistic回归分析结果表明,飞行人员年龄每增加1岁,高甘油三酯患病风险改变1.072倍(95%CI:1.049~1.096,P<0.001)。飞行时间1000~2000 h的飞行人员高甘油三酯患病风险是飞行时间≤1000 h飞行人员的1.997倍(95%CI:1.579~2.525,P<0.001);飞行时间>2000 h的飞行人员高甘油三酯患病风险是飞行时间≤1000 h飞行人员的2.664倍(95%CI:1.860~3.815,P<0.001)。陆军飞行人员高甘油三酯患病风险是海军的0.775倍(95%CI:0.618~0.971,P=0.027);空军飞行人员高甘油三酯患病风险是海军的0.669倍(95%CI:0.475~0.942,P=0.021)。预警机飞行人员高甘油三酯患病风险是轰炸机飞行人员的2.710倍(95%CI:1.292~5.683,P=0.008);直升机飞行人员高甘油三酯患病风险是轰炸机飞行人员的0.666倍(95%CI:0.467~0.951,P=0.025)。结论近5年来,飞行人员高甘油三酯血症患病趋势增加,随着年龄增长和飞行时间延长飞行人员患病风险增加;海军飞行人员、预警机飞行人员患病风险高。疗养院要针对危险因素进行干预,控制疾病发展,维护飞行人员身体健康。Objective To investigate and analyze the prevalence and risk factors of hypertriglyceridemia among flight personnel by retrospectively studying the medical examination data so as to contribute to health management in a sanatorium. Methods The physical examination data of flight personnel who had undergone comprehensive checkups between 2013 and 2017 was collected. Statistical methods were used to analyze the prevalence and risk factors of hypertriglyceridemia. Results( 1) Between2013 and 2017,the prevalence of hypertriglyceridemia tended to increase( P〈0.001). Over those five years,the average incidence was 17.2%. The prevalence in 2017 was 2.1 times that in 2013.( 2) The average age of flight personnel in the case group was( 36.6±7.6) and their average flight time was( 2455±1215) hours,both of which were significantly above those of the control group( P〈0.001).( 3) The results of Spearman analysis showed that the risk of hypertriglyceridemia in flight personnel was positively correlated with age( r = 0.361,P〈0.001) and flight time( r = 0.377,P〈0.001),but negatively with marital status( unmarried)( r =-0.128,P〈0.001).( 4) Multivariate logistic regression analysis showed that the risk of hypertriglyceridemia increased by 1.072 times with the increase of one year in age among flight personnel( 95% CI: 1.049 to 1.096,P〈0.001).The risk of hypertriglyceridemia among flight personnel with a flight time of 1,000 to 2,000 hours was 1.997 times that of flight personnel with a flight time less than or equal to1000 hours. Flight personnel who had a fight time of over 2000 hours was at 2.664 times as high a risk of hypertriglyceridemia as those with less than 1,000 flight hours( 95%CI: 1.860 - 3.815,P〈0.001).Army pilots were 0.775 times as vulnerable to high triglycerides as Navy pilots( 95%CI: 0.618 - 0.971,P = 0.027).Air Force pilots were 0.669 times as likely to suffer from high triglycerides as Navy pilots( 95%CI: 0.475 - 0.942,P = 0.02
