摘  要:目的:通过研制充气式气管套管固定带并应用于临床气管切开患者,探讨其能否有效解决现有改良气管套管固定带存在的一些不足之处。方法:随机选取2016年3月-2017年3月中我院ICU中气管切开留置气管套管的患者100例,随机分为两组,其中对照组患者50名,采用传统改良气管套管固定带固定,实验组患者50名,采用自制充气式气管套管固定带固定。比较两组患者不良事件发生情况、气管脱出和固定带更换次数和各项情况所需时间。结果:实验组不良情况中Ⅰ度皮损(χ2=10.84,P=0.00)、Ⅱ度皮损(χ2=12.83,P=0.00)、Ⅲ度皮损(χ2=7.44,P=0.00)、咳嗽(χ2=12.29,P=0.00)、气促(χ2=8.36,P=0.00)要远远少于对照组,实验组气管移位(χ2=11.12,P=0.00)、部分脱出(χ2=10.19,P=0.00)、完全脱出(χ2=6.57,P=0.00)均远远少于对照组,实验组更换1次(χ2=16.06,P=0.00)和超过1次(χ2=11.15,P=0.00)固定带与对照组的比较差异具有统计学意义。实验组的固定带使用时间(t=27.11,P=0.00)高于对照组,每次更换固定带所需要的时间(t=11.08,P=0.00)少于对照组。出现皮损的时间(t=18.74,P=0.00)晚于对照组。结论:自制充气式气管套管固定带能有效减少患者护理过程中不良情况,减少对患者的刺激,值得临床推广。Objective To explore the efcacy of infatable fxed with tracheal cannula on clinical tracheotomy. Methods: 100 patients were randomly divided into two groups. 50 patients in the control group were treated with traditional modifed tracheal cannula fxation. 50 patients in the experimental group were treated with infatable fxed with tracheal cannula. The incidence of adverse events, the number of tracheal prolapse and the number of fxed belt replacement and the required time for each condition were compared between two groups. Results: In the experimental group, the Ⅰ degree lesion, Ⅱ degree lesion, Ⅲ degree lesion, cough and shortness of breath were far less than the control group. The tracheal displacement, partial prolapse and complete prolapse were signifcantly less than the control group. There was statistically signifcant diferences in number of fxed belt replacement between the experimental group and the control group. The applied time of fxed belt in the experimental group was higher than the control group. Replace the fxed time in the experimental group was shorter than the control group. The time of skin lesions in the experimental group was later than the control group. Conclusion: The infatable fxed with tracheal cannula can efectively reduce the adverse circumstances in nuring process, reduce the stimulation of patients, and is worthy of clinical promotion.
