比较不同部位皮瓣移植对于颜面部不同类型皮肤软组织缺损的治疗疗效

摘　　要：目的比较不同部位皮瓣移植对于颜面部不同类型皮肤软组织缺损的治疗疗效。方法方便选取该院2016年10月—2017年10月收治的84例皮肤软组织缺损需要实施皮瓣移植及皮肤软组织扩张术的患者,按照取皮部位不同分为面部组与胸部组,各42例。面部组采用面部皮肤软组织扩张术与皮瓣移植,胸部组采用胸部皮肤软组织扩张术与皮瓣移植,对比观察两组患者在术后的皮肤颜色、毛发生长情况以及皮肤外观等的变化,另外还需观察两组的不良反应发生率。结果胸部组的皮肤颜色评分为(84.61±10.76)分,面部组为(82.36±11.26)分,差异无统计学意义(t=0.936,P=0.352)。胸部组的毛发生长情况评分为(79.36±13.34)分,面部组为(80.26±10.47)分,差异无统计学意义(t=0.344,P=0.732)。胸部组的皮肤外观评分为(85.37±11.83)分,面部组为(81.47±16.38)分(t=1.251,P=0.215)。胸部组中发生不良反应的有4例(9.52%),面部组中发生不良反应的有5例(11.90%)(χ~2=0.124 4,P=0.724)。结论在对皮肤软组织缺损需要进行皮肤软组织扩张术以及皮瓣移植的患者进行治疗时,采取面部或是胸部皮肤进行软组织扩张术或是皮瓣移植的治疗效果并无显著差异,不良反应的发生率也无显著差异,应针对患者的具体情况采取相应的治疗方法。Objective To compare the therapeutic effects of skin grafts of different sites on different types of skin and soft tissue defects. Methods Convenient select 84 patients with skin and soft tissue defects admitted to the hospital from October 2016 to October 2017 were requited to perform skin flap transplantation and skin soft tissue expansion. The patients were divided into two groups： face group and chest group, 42 eases each. The facial group was treated with facial skin soft tissue expansion and skin flap transplantation. The chest group was treated with chest skin soft tissue expansion and flap transplantation. The changes in skin color, hair growth, and skin appearance of the two groups after the operation were observed. Observe the incidence of adverse reactions in both groups. Results The skin color score of the thoracic group was （84.61± 10.76）points, and the facial group was （82.36±11.26）points. The difference was not statistically significant （t=0.936, P= 0.352）. The hair growth score of the chest group was （79.36±13.34）points, and the facial group was （80.26±10.47）points. The difference was not statistically significant （t=0.344, P=0.732）. The skin appearance score of the chest group was （85.37±11.83）points, and the facial group was （81.47±16.38）points （t=1.251, P=0.215）. 4 patients （9.52%） developed adverse reactions in the chest and 5 patients （11.90%） developed adverse reactions in the face group （χ2=0.124 4, P=0.724）. Conclusion In the treatment of patients with skin and soft tissue defects requiring skin soft tissue expansion and skin flap transplantation, there was no significant difference in the effect of soft skin. There was also no significant difference, and appropriate of the patient. tissue expansion or flap transplantation on the face or chest treatment should be taken according to the specific conditions of the patient.
