老年股骨粗隆间骨折患者行闭合复位髓内针内固定的临床疗效

摘　　要：目的观察并探讨闭合复位髓内针内固定治疗老年股骨粗隆间骨折患者的临床效果。方法选取2017年3月—2018年3月期间来该院就诊的老年股骨粗隆间骨折患者128例,随机分为实验组(闭合复位髓内针内固定治疗)与对照组(切开复位髓外固定治疗),每组患者人数相同各64例,对两组患者的术中出血量、手术时间、术后功能评分、并发症发生情况与治疗优良率进行观察与比较。结果实验组及对照组患者治疗优良率分别为95.31%、78.12%(χ~2=3.241 4,P=0.008),实验组及对照组患者治疗后并发症发生率分别为1.56%、12.50%(χ~2=2.8547,P=0.006),实验组患者术中出血量、手术时间、术后功能评分分别为(140.37±8.54)mL、(58.32±3.24)min、(6.48±0.57)分,对照组患者术中出血量、手术时间、术后功能评分分别为(191.34±11.24)mL、(82.54±4.38)min、(4.97±0.44)分(t=2.576、1.358、2.021,P=0.004、0.006、0.008)。实验组患者术中出血量、手术时间、术后功能评分、治疗优良率及治疗后并发症发生率均显著优于对照组,两组间差异有统计学意义(P<0.05)。结论老年股骨粗隆间骨折患者行闭合复位髓内针内固定治疗具有显著的临床疗效,能够有效促进患者的功能恢复,值得推广应用。Objective To observe and investigate the clinical effects of closed reduction and intramedullary nailing in the treatment of elderly patients with intertrochanteric fractures. Methods A total of 128 patients with intertrochanteric fractures of the elderly who came to the hospital from March 2017 to March 2018 were randomly divided into the experimental group （closed reduction and intramedullary internal fixation） and the control group （open reduction）. For extramedullary fixation, the number of patients in each group was the same in 64 cases. The intraoperative blood loss, operation time, postoperative functional scores, occurrence of complications, and excellent rate of treatment were observed and compared between the two groups. Results The excellent rate of treatment in the experimental group and control group were 95.31% and 78.12%（χ2=3.241 4,P=0.008）. The incidence of post-treatment complications in the experimental group and control group were 1.56% and 12.50%, respectively（χ2=2.854 7,P=0.006）. The intraoperative blood loss, operation time, and postoperative functional score in the experimental group were （140.37±8.54）mL, （58.32±3.24） min, （6.48±0.57） points, respectively, and the amount of intraoperative blood loss in the control group. The operation time and postoperative functional score were （191.34±11.24）mL, （82.54±4.38）min, and （4.97±0.44） points respectively.（t=2.576, 1.358, 2.021,P=0.004, 0.006, 0.008）. The intraoperative blood loss, operation time, postoperative functional score, excellent treatment rate and postoperative complication rate were significantly better in the experimental group than in the control group. The difference between the two groups was statistically significant（P〈0.05）. Conclusion Closed reduction intramedullary nail fixation in elderly patients with intertrochanteric fracture has significant clinical curative effect and can effectively promote the functional recovery of patients.
