摘  要:目的分析超声诊断胎儿先天性肺囊腺瘤样畸形(CCAM)的声像图特征。方法分析我院产前超声诊断为CCAM的72例胎儿超声声像图特征,并对照胎儿结局(引产及分娩结果)评价超声诊断符合率。结果Ⅰ型CCAM胎儿肺动脉分支来源血供未探及率及CVR<1.6比例均为100.00%,明显高于Ⅱ型、Ⅲ型(P <0.05)。Ⅱ型CCAM胎儿肺动脉分支来源血供未探及率均高于Ⅲ型(P <0.05)。Ⅰ型囊腔平均直径明显高于Ⅱ型、Ⅲ型(P <0.05),Ⅱ型囊腔平均直径明显高于Ⅲ型(P <0.05)。Ⅰ型CCAM胎儿无胎儿水肿等合并症;Ⅱ型胎儿水肿、纵隔受压发生率均为21.88%,均高于Ⅰ型(P <0.05);Ⅲ型胎儿水肿、纵隔受压发生率均为50.00%,均高于Ⅰ型、Ⅱ型(P <0.05)。以胎儿结局作为标准,Ⅰ型、Ⅱ型、Ⅲ型超声诊断符合率分别为100.00%、100.00%、 90.91%。结论超声诊断胎儿CCAM的准确性较高,是产前诊断CCAM的首选方法。Objective To analyze the sonographic characteristics of ultrasonography in the diagnosis of fetal congenital cystic adenomatoid malformation(CCAM). Methods The sonographic characteristics of 72 cases of fetuses diagnosed as CCAM in our hospital were analyzed, and the diagnostic coincidence rate of ultrasonography was evaluated compared with the fetal outcomes(induced labor and delivery outcome). Results The unexplored rate of blood supply from pulmonary artery branch and the rate of CVR 1.6 of type Ⅰ CCAM were both 100.00%, significantly higher than the type Ⅱ and type Ⅲ(P 0.05). The unexplored rate of blood supply from pulmonary artery branch and the rate of CVR 1.6 of type Ⅱ were higher than type Ⅲ(P 0.05). The average diameter of type Ⅰ cysts was significantly higher than type Ⅱ and type Ⅲ(P 0.05). The average diameter of type Ⅱ cysts was significantly higher than type Ⅲ(P 0.05). Type Ⅰfetuses had no complications, such as fetal edema. The incidences of fetal edema and mediastinum compression of type Ⅱ were both21.88%, higher than type Ⅲ(P 0.05). The incidences of fetal edema and mediastinum compression of type Ⅲ were both 50.00%, higher than type Ⅰ and type Ⅱ(P 0.05). Taking fetal outcomes as the standard, the diagnostic coincidence rates of ultrasonography of type Ⅰ,type Ⅱ and type Ⅲ were 100.00%, 100.00% and 90.91% respectively. Conclusions Ultrasonography in the diagnosis of fetal CCAM has higher accuracy, which is the first choice for prenatal diagnosis of CCAM.
