摘 要:目的探讨C反应蛋白、降钙素原和血小板计数在新生儿败血症诊断中的临床应用效果。方法选取我院2015年9月至2018年2月期间收治的143例败血症新生儿作为败血症组,选取同期99例局部感染新生儿作为感染组,选取同期132例健康新生儿作为对照组。分析比较三组新生儿的C反应蛋白、降钙素原和血小板计数水平。结果与感染组和对照组相比,败血症组的C反应蛋白和降钙素原水平均较高,血小板计数较低,差异均有统计学意义(P <0.05)。联合诊断新生儿败血症的灵敏度、特异度、阳性预测值和阴性预测值均高于C反应蛋白、降钙素原和血小板计数单独诊断(P <0.05)。结论新生儿败血症患儿的C反应蛋白、降钙素原和血小板计数会发生显著变化,三者联合检测可作为临床诊断新生儿败血症的观测指标。Objective To explore the clinical application effect of C-reactive protein, procalcitonin and platelet count in neonatal septicemia. Methods 143 cases of neonates with septicemia admitted to our hospital from September 2015 to February 2018 were selected as the septicemia group, 99 cases of neonates with local infection during the same period were selected as the infection group, and 132 healthy neonates during the same period were selected as the control group. The C-reactive protein, procalcitonin and platelet count levels were analyzed and compared among three groups. Results Compared with the infection group and control group, the C-reactive protein and procalcitonin levels of the septicemia group were higher, and the platelet count was lower(all P 0.05). The sensitivity, specificity, positive predictive value and negative predictive value of combined diagnosis of neonatal septicemia were higher than those of single diagnosis of C-reactive protein, procalcitonin and platelet counts(P 0.05). Conclusions The C-reactive protein, procalcitonin and platelet counts of neonates with septicemia will significantly change, and the combined detection of the three can be used as an observational indicator for clinical diagnosis of neonatal septicemia.
