摘 要:目的通过间接免疫荧光法检测九种呼吸道感染病原体IgM抗体,分析北京地区患呼吸道系统疾病患者病原体分布情况及流行病学特点,为呼吸道病原体的预防控制提供理论依据。方法回顾性分析解放军总医院14383例患者,应用间接免疫荧光法(IFA)检测九项呼吸道感染病原体IgM抗体,比较其在不同年龄段、季节的分布情况和流行病学特点,对病原体感染分布情况进行统计分析。结果北京地区2013-2016年呼吸道疾病患者呼吸道病原体抗体的总阳性率为24.31%(3496/14383),病原体抗体主要为肺炎支原体抗体,检出率为18.25%、乙型流感病毒抗体,检出率为7.65%和副流感病毒抗体,检出率为2.30%,单一感染占80.12%;各年龄组肺炎支原体、乙型流感病毒、副流感病毒、腺病毒病原体抗体阳性检出率比较,差异有统计学意义(P<0.05),少年组高于其他年龄组;不同季节肺炎支原体、乙型流感病毒和腺病毒抗体检出率比较,差异有统计学意义(P<0.05),其中春、冬两季的肺炎支原体检出率高于夏、秋两季。结论通过IFA检测可初步掌握北京地区呼吸道系统感染的流行病学特点和发病率,并为临床诊断和治疗提供参考。OBJECTIVE To detect the IgM antibodies of nine species of pathogens causing respiratory tract infections through indirect immunofluorescence assay and investigate the distribution of pathogens and epidemiological characteristics of patients with respiratory tract infection in Beijing so as to provide theoretical basis for prevention and control of the respiratory tract pathogens.METHODS A total of 14383 patients who were treated in Chinese PLA General Hospital were retrospectively analyzed,the IgM antibody of nine species of respiratory tract pathogens was detected by using indirect immunofluorescence assay(IFA).The distribution of the pathogens in age groups and seasons and the epidemiological characteristics were observed and compared,and the distribution of the patients with pathogens infections was statistically analyzed.RESULTS The total positive rate of antibody of the respiratory tract pathogens was 24.31%(3496/14383)among the patients with respiratory tract diseases in Beijing from 2013 to 2016.The detection rate of antibody of Mycoplasma pneumoniae was 18.25%,the detection rate of antibody of influenza B virus was 7.65%,and the detection rate of antibody of parainfluenza virus was 2.30%.The patients with single infection accounted for 80.12%.There were significant differences in the positive rates of antibodies of M.pneumoniae,influenza B virus,parainfluenza virus and adenovirus among the age groups(P〈0.05);the positive rates of the teenager group were higher than those of other age groups.There were significant differences in the detection rates of antibodies of M.pneumoniae,influenza B virus and adenovirus among the seasons(P〈0.05);the detection rate of antibody of M.pneumoniae was higher in spring and winter than in summer and autumn.CONCLUSIONThe IFA may facilitate the preliminary understanding of the epidemiological characteristics and morbidity rates of respiratory tract infection in Beijing and provide guidance for clinical diagnosis and treatment.
