开塞露纳肛预防脐血移植患者肛周感染的应用研究

摘　　要：目的探讨实施开塞露纳肛辅助通便预防恶性血液病行非血缘脐血移植患者肛周感染的应用效果。方法采用方便抽样法,选取医院58例恶性血液病行非血缘脐血移植患者作为研究对象,按住院时间段分为试验组(实施开塞露纳肛辅助通便预防便秘)30例;对照组(采用常规肛周护理流程)28例。比较两组患者肛周感染、血流感染发生情况。结果 30d肛周感染发生率试验组和对照组分别为23.33%和42.86%,试验组肛周感染发生率与对照组比较有降低趋势,但差异无统计学意义(P=0.132);实试验组无3度肛周感染发生,对照组有3例发生3度肛周感染;试验组血流感染发生率为10.00%(3/30),低于对照组血流感染率32.14%(9/28),两组比较差异有统计学意义(P=0.038)。结论实施开塞露纳肛辅助通便预防便秘可降低恶性血液病脐血移植患者肛周感染的发生率和肛周感染程度,可有效降低血流感染的发生。OBJECTIVE To explore the effect of glycerine enema on prevention of perianal infection in hematologic malignancies patients undergoing unrelated cord blood transplantation.METHODS By means of convenient sampling,58 patients with hematologic malignancies who underwent the unrelated umbilical cord blood transplantation were recruited as the study objects and were divided into the experimental group with 30 cases（the patients who were treated with glycerine enema）and the control group with 28 cases（the patients who were treated with conventional perianal nursing）according to time segments of hospitalization.The incidence rates of perianal infection and bloodstream infection were observed and compared between the two groups of patients.RESULTS The 30-day incidence rate of perianal infection was 23.33%in the experimental group,42.86%in the control group;the incidence rate of perianal infection of the experimental group was significantly lower than that of the control group（P=0.132）.There was no case of grade 3 perianal infection in the experimental group and 3 cases of grade 3 perianal infection in the control group.The incidence rate of bloodstream infection of the experimental group was 10.00%（3/30）,significantly lower than 32.14%（9/28）of the control group（P=0.038）.CONCLUSIONThe glycerine enema may reduce the incidence of perianal infection in the hematologic malignancies patients undergoing the cord blood transplantation and the degree of perianal infection as well as the incidence of bloodstream infection.
