脓毒症患儿血清CRP与PCT水平变化的临床意义

摘　　要：目的分析C-反应蛋白(CRP)、降钙素原(PCT)水平变化对脓毒症患儿临床诊断的价值。方法选取医院2016年3月-2017年3月收治的52例脓毒症患儿作为研究组,选择同期50例非脓毒症感染患儿作为对照组,分析两组患儿感染病原学特点及治疗前后患儿血清PCT、CRP和血WBC水平变化情况。结果两组患儿感染病原菌种类比较差异无统计学意义(χ2=0.221,P=0.479);治疗前,研究组患儿PCT、CRP和WBC水平高于对照组,差异有统计学意义(P<0.05);治疗后1、5、10d,研究组PCT、CRP及WBC水平均高于对照组,差异有统计学意义(F=10.223,8.223,6.223;P均<0.001)。结论血清CRP、PCT水平可作为脓毒症感染患儿的早期诊断标准,对评估患儿病情的严重程度和临床诊断具有十分重要的意义。OBJECTIVE To analyze the clinical value of serum C-reactive protein（CRP）and procalcitonin（PCT）in clinical diagnosis of children with sepsis.METHODS Totally 52 children with sepsis who were treated in the hospital from Mar 2016 to Mar 2017 were assigned as the study group,meanwhile,50 children without sepsis were chosen as the control group.The etiological characteristics of sepsis and the levels of serum CRP,PCT and WBC were observed and compared between the two groups of children before and after the treatment.RESULTS There were no significant differences in the species of pathogens between the two groups of children（χ^2=0.2213,P=0.479）.The levels of PCT,CRP and WBC of the study group were significantly higher than those of the control group before the treatment（P〈0.05）.The levels of PCT,CRP and WBC of the study group were significantly higher than those of the control group after the treatment for 1,5 and 10 days（F=10.223,8.223,6.223;all P〈0.001）.CONCLUSIONThe levels of serum CRP and PCT can serve as the criteria for early diagnosis of the children with sepsis and have great significance in assessment of severity of illness and clinical diagnosis.
