老年痴呆患者肺部感染相关因素分析与护理对策

摘　　要：目的研究老年痴呆患者发生肺部感染的影响因素及护理干预措施。方法选择2012年2月-2015年12月医院收治的152例老年痴呆患者为研究对象,采用回顾性病例研究方法,观察纳入研究对象的临床资料,分析可能导致肺部感染的相关因素,提出针对性护理建议。结果 152例老年痴呆患者,发生肺部感染患者26例,感染率为17.11%;老年痴呆患者并发感染与长期卧床、滥用抗菌药物、合并糖尿病、病程时间、进食障碍、低蛋白血症、吸烟史有关(P<0.05),多因素Logistic回归分析显示,滥用抗菌药物、合并糖尿病、病程时间、低蛋白血症、吸烟史是老年痴呆患者并发肺部感染的独立相关因素。结论老年痴呆患者并发肺部感染与滥用抗菌药物、合并糖尿病、病程时间、低蛋白血症、吸烟史有关,临床对老年痴呆患者治疗及护理过程中,应结合肺部感染的相关因素,针对性制定护理措施。OBJECTIVE To explore the influencing factors for pulmonary infections in senile dementia patients and put forward the nursing countermeasures.METHODS A total of 152 senile dementia patients who were treated in the hospital from Feb 2012 to Dec 2015 were recruited as the study objects,the clinical data of the enrolled patients were observed by means of retrospective survey,the related factors for the pulmonary infection were analyzed,and the targeted nursing countermeasures were put forward.RESULTS Of the 152 senile dementia patients,26 had pulmonary infections,with the infection rate 17.11%.The incidence of the infections in the senile dementia patients was associated with the long bedridden time,abuse of antibiotics,complication with diabetes mellitus,disease course,eating disorder,hypoproteinemia and smoking history（P〈0.05）.Multivariate logistic regression analysis showed that the abuse of antibiotics,complication with diabetes mellitus,disease course,hypoproteinemia and smoking history were the independent related factors for the pulmonary infection in the senile dementia patients.CONCLUSIONThe incidence of the pulmonary infections in the senile dementia patients is associated with the abuse of antibiotics,complication with diabetes mellitus,disease course,hypoproteinemia and smoking history.It is necessary for the hospital to put forward the targeted nursing countermeasures according to the related factors for the pulmonary infections during the treatment and nursing of the senile dementia patients.
