健脾益肺颗粒抑制肺脾气虚COPD患者气道炎症机制研究

摘　　要：【目的】观察健脾益肺颗粒对慢性阻塞性肺疾病(COPD)肺脾气虚型患者痰液中组蛋白去乙酰化酶(HDACs)活性与表达及炎症细胞因子[主要包括白细胞介素-6(IL-6)、白细胞介素-8(IL-8)]水平的影响。【方法】根据中医辨证分型标准,将37例COPD稳定期患者分为3组,肺气虚组14例,肺脾气虚组13例,肺肾气虚组10例。比较不同中医证型与HDACs活性与表达及IL-6、IL-8水平关系;并对肺脾气虚型患者进行为期3个月治疗,观察治疗前后HDACs活性与表达及IL-6、IL-8水平变化情况。【结果】(1)肺气虚、肺肾气虚、肺脾气虚3组不同中医证型COPD稳定期患者痰液中HDACs活性与蛋白表达及IL-6、IL-8水平比较,差异均无统计学意义(P>0.05)。(2)肺脾气虚型患者治疗后痰液中HDACs活性与蛋白表达较治疗前明显升高(P<0.05),而炎症细胞因子IL-6、IL-8水平较治疗前明显下降(P<0.05)。【结论】健脾益肺颗粒治疗肺脾气虚型COPD患者,可升高HDACs活性与表达,降低IL-6和IL-8水平,从而改善患者的肺部炎症。Objective The effects of Jianpi Yifei Granules（JYG） on the activity and expression of histone deacetylases（HDACs）as well as the levels of inflammatory cytokines including interleukin（IL）-6,IL-8 in the sputum of chronic obstructive pulmonary disease（COPD）patients with lung-spleen Qi deficiency syndrome.Methods According to results of traditional Chinese medicine（TCM）syndrome differentiation,37 cases of COPD patients at stable stage were divided into 3 groups,namely lung Qi deficiency syndrome group（14 cases）, lung-spleen Qi deficiency syndrome group（13 cases）and lung-kidney Qi deficiency syndrome group（10 cases）. The relationship of TCM syndrome types with the expression and activity of HDACs and the levels of IL-6,IL-8 were compared. The patients with lung-spleen Qi deficiency syndrome were treated with JYG for 3 months. The changes of the expression and activity of HDACs and the levels of IL-6,IL-8 were observed before and after treatment.Results（1）There was no significant difference in the expression and activity of HDACs and the levels of IL-6 and IL-8 between the 3 groups（P〉0.05）.（2）The expression and activity of HDACs were ameliorated and the levels of IL-6,IL-8 were decreased significantly in the lung-spleen Qi deficiency syndrome group after treatment as compared with those before treatment（P〈0.05）. Conclusion JYG can ameliorate the inflammation in COPD patients with lung-spleen Qi deficiency syndrome through increasing the expression and activity of HDACs and decreasing the levels of IL-6 and IL-8.
