雷火灸联合真武汤加减治疗原发性肾病综合征疗效观察

摘　　要：【目的】探讨雷火灸联合真武汤加减治疗脾肾阳虚型肾病综合征(PNS)的临床疗效。【方法】将60例脾肾阳虚型PNS患者随机分为联合组和西药组,每组各30例。西药组给予标准化激素疗法和一般常规治疗,联合组在西药组治疗的基础上加用雷火灸(取穴:中脘、关元、水道)和真武汤加减治疗。治疗3个月后评价2组临床疗效,比较2组患者治疗前后的临床症状、体征及24 h尿蛋白定量、血清白蛋白(Alb)及血脂[总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)]水平的变化情况。【结果】(1)经1个疗程治疗后,联合组总有效率为93.33%,西药组为70.00%;联合组的疗效明显优于西药组,差异有统计学意义(P<0.05)。(2)治疗后,2组患者24 h尿蛋白定量、TC、LDL-C、中医证候积分均较治疗前降低(P<0.05或P<0.01),Alb水平均较治疗前升高(P<0.01),且联合组在降低24 h尿蛋白定量、TC、LDL-C、中医证候积分和提高Alb水平方面均优于西药组,差异均有统计学意义(P<0.05)。【结论】雷火灸联合真武汤加减治疗PNS,能明显改善患者临床症状,有效降低患者24 h尿蛋白定量和血脂TC、LDL水平,提高患者血清Alb水平,疗效显著。Objective To explore clinical curative effect of thunder fire moxibustion combined with Zhenwu Decoction for primary nephrotic syndrome. Methods A total of 60 cases of primary nephrotic syndrome patients with spleen and kidney yang deficiency syndrome were randomly divided into combined group（30 cases）and western medicine group（30 cases）. The western medicine group and combined group received standardized hormone therapy and routine treatment,and additionally,the combined group received thunder fire moxibustion and modified Zhenwu Decoction treatment. After treatment for 3 months,the curative effect in the two groups wase valuated,and clinic symptoms and signs,24 h urinary protein quantitation,the levels of serum albumin and blood lipids including total cholesterol（TC）,triglyceride（TG）,low density lipoprotein cholesterol（LDL-C）were compared. Results（1）After one treatment course,the total effective rate was 93.33% in the combined group,was 70.00% in the western medicine group,and the curative effect in the combined group was superior to that in the western medicine group（P〈0.05）.（2）After treatment,24 h urinary protein quantitation,TC,LDL-C and traditional Chinese medicine（TCM）syndrome scores in the two groups were significantly lower than those before treatment（P〈0.05 or P〈0.01）,the serum Alb was significantly higher（P〈0.05）;the improvement effect in the combined group was superior to that in the western medicine group,the difference being significant（P〈0.05）.Conclusion Thunder fire moxibustion combined with Zhenwu Decoction can improve clinical symptoms of nephrotic syndrome patients,effectively reduce 24 h urinary protein quantitation and the levels of TC and LDL-C,and increase serum albumin,showing significant curative effect.
