维持性血液透析患者血清成纤维细胞生长因子23与骨质疏松相关性研究

摘　　要：目的观察维持性血液透析(MHD)患者成纤维细胞因子(FGF)23水平,并探讨其与骨质疏松、骨密度及其他骨代谢指标的关系。方法选择2016年6月~2017年6月于潍坊市人民医院血液净化中心行规律血液透析治疗的患者80例作为研究对象,并选取40例健康体检者作为对照组,采用酶联免疫方法检测其血FGF23水平,并采用双能X线骨密度仪测定股骨干骨密度,同时收集患者的一般资料、生化指标,对各参数进行统计学分析。结果纳入研究的80例维持性血液透析患者中,骨密度异常组占MHD总人数的72.50%;而对照组中骨量异常组占对照组总人数的7.50%;FGF23水平与血甲状旁腺激素、血尿素氮、血钙、血磷间均存在正相关(P<0.05),而与骨质疏松程度、骨密度及其他指标间无明显相关性。结论维持性血液透析患者骨质疏松发生率明显高于正常人;血清FGF23虽对骨代谢水平有早期预测作用,但透析后骨质疏松发生机制比较复杂,可能未受到FGF23水平的直接影响。Objective To observe serum fibroblast growth factor 23 levels of the maintenance hemodialysis patients,and to explore its relationship with osteoporosis,bone mineral density and other indicators of bone metabolism. Methods According to inclusion and exclusion criteria,from June 2016 to June 2017,80 patients with nemodialysis in blood purification center of Weifang People＇s Hospital were seleted,and 40 cases of healthy physical examination were selected as control group. Serum FGF23 levels were measured by enzyme-linked immunosorbent assay,femoral bone mineral density determined by dual energy X-ray absorptiometry,and general biochemical indicators of patients were collected. The parameters were analyzed by statistics. Results Among the 80 maintenance hemodialysis patients included in the study,the bone density abnormalities group accounted for 72. 50 of the total number of MHD; while the abnormal bone mass in the control group accounted for 7. 50% of the total number of controls. The level of FGF23 was positively correlated with parathyroid hormone,blood urea nitrogen,serum calcium and serum phosphorus（ P〈0. 05）,but not with the degree of osteoporosis,bone mineral density and other indexes. Conclusion Osteoporosis in maintenance hemodialysis patients is significantly higher than normal. Although serum FGF23 has an early predictive value on bone metabolism,the mechanism of osteoporosis after dialysis is complicated and may not be directly affected by the level of FGF23.
