摘 要:目的调查攀枝花地区老年尿路感染患者的肠球菌分类、临床分布及耐药情况,为攀西地区合理选用抗生素治疗老年肠球菌尿路感染提供依据。方法对老年尿路感染患者尿培养检出的216株肠球菌行回顾性分析,应用Whonet 5.6软件及SPSS 15.0统计软件进行数据分析。结果 216株肠球菌主要来源于泌尿外科(40.28%)和重症监护病房(28.70%)。肠球菌属主要为粪肠球菌136株(62.96%)和屎肠球菌75株(34.72%);体外药敏显示肠球菌对青霉素、氨苄青霉素、红霉素、左氧氟沙星、高浓度庆大霉素及高浓度链霉素的总耐药率均超过>60%,对青霉素及氨苄青霉素的耐药率逐年上升;对呋喃妥因、万古霉素及利奈唑胺耐药率低,分别为17.13%、1.39%和0.00%。共检出3株屎肠球菌对万古霉素耐药。结论攀西地区老年患者肠球菌尿路感染的细菌耐药率高,患者主要分布于泌尿外科和重症监护病房。肠球菌仅对呋喃妥因、万古霉素及利奈唑胺敏感,临床应加强抗生素合理用药。Objective To investigate the distribution and drug resistance of Enterococci in old patients with urinary tract infection and provide evidence for the rational use of antibiotics. Methods Distribution and drug resistance of 216 Enterococci coming from old patients with urinary tract infection were investigated by retrospective method. WHONET 5.6 software and SPSS 15.0 software were used for data analysis. Results A total of 216 Enterococcus strains were detected. The strains were mainly detected in Department of Urology (40.28%) and Department of Intensive Care Unit (28.70%). The strains include 136 strains of E.faecalis (62.96%) and 75 strains of E.faecium (34.72%). The resistant rate of E.faecium and E. faecalis to Ampicllin, Penicillin, high-level Gentamicin, Rifampicin, Erythromycin and Levofloxacin both exceeded 60.00%. The resistant rate of E. faecalis to Furadantin, vancomycin, and linezolid were 17.13%, 1.39% and 0.00%. The resistant rate of E. faecalis and E.faecalis were different. There are three E.faecium resistance to vancomycin. Conclusion The resistant rates of the Enterococcus were high to polyantibiotic in old people coming from Panzhihua. Enterococcus is only sensitive to furoflodone, vancomycin and linezolid. Antibiotics should be used rationally.
