摘　　要：目的探析慢性病医保制度发展与改革对慢性病患者医从性的影响作用。方法选取2016年1月至2017年6月入我院门诊接受治疗的130例慢性病患者作为研究对象,所有纳入病例病程≥10年,入院就诊中经历过慢性病医保制度改革,从纵向医保制度是否改革为标准分为政策前、政策后两个组别,观察政策前后患者年就诊次数及年就诊率,统计年均费用花销变化情况,分析慢性病医保制度对慢性病患者医从性的影响。结果慢性病医保制度政策变化后,门诊部分费用可报销,患者年就诊次数、年就诊率均高于政策前(P<0.05),医从性显著改善。结论慢性病医保制度改革的方向应以门诊统筹对策为切入点,增加医疗补贴范围及费用份额,真正减轻患者及其家庭的经济负担,提升患者医从性,进而良好控制病情,提高患者生活质量。Objective To explore the impact of the development and reform of the medical insurance system on the medical quality of patients with chronic diseases. Methods From January 2016 to June 2017, 130 cases of chronic diseases treated in the outpatient department of our hospital were selected as the research object. The duration of all the included cases was over 10 years. The medical insurance system under chronic diseases was admitted to the hospital during the period of admission. From the longitudinal medical insurance system whether the reform is the standard is divided into two groups before the policy and the policy after the policy, observing the annual number of visits and the annual visiting rate of the patients before and after the policy, the statistical annual average cost of spending changes, analysis of chronic diseases medical insurance system to patients with chronic diseases. Results After the change of policy of chronic medical insurance policy, the expenses of outpatient part can be reimbursed. The number of annual visiting and annual visiting rate of patients is higher than that before the policy（P〈0.05）. Conclusion The direction of the reform of the medical insurance system for chronic diseases should take the out-patient co-ordination measures as an entry point, increase the scope of medical subsidies and the share of costs, and truly reduce the financial burden on patients and their families, improve the quality and control the disease and improve the quality of life of patients.
