保阴煎对早期先兆流产患者血清孕酮、人绒毛膜促性腺激素、癌抗原125的影响

摘　　要：目的:探讨保阴煎联合地屈孕酮片治疗早期先兆流产的临床疗效。方法:选取2016年7月至2018年1月重庆三峡医药高等专科学校附属医院与深圳市中医院妇产科门诊及住院部收治的早期先兆流产患者60例,随机分为对照组与观察组,每组30例,对照组给予口服地屈孕酮片,观察组在对照组治疗基础上加用口服保阴煎中药汤剂,2组均治疗3周。比较2组临床主要症状积分及总积分、临床疗效及血清孕酮(P)、β-人绒毛膜促性腺激素(β-HCG)、癌抗原125(CA125)水平。结果:观察组临床疗效有效率为93.3%,与对照组有效率80.0%比较,差异有统计学意义(P<0.05);观察组临床症状阴道出血、小腹疼痛或坠胀、腰酸胀痛积分改善明显优于对照组(P<0.05);治疗后2组患者血清P、β-HCG水平显著升高,观察组显著高于对照组(P<0.05);治疗后2组患者血清CA125显著降低,观察组与对照组比较,差异有统计学意义(P<0.05)。结论:保阴煎联合地屈孕酮片治疗早期先兆流产能提高临床疗效,提升患者血清P、β-HCG水平,下调血清CA125水平,降低流产风险,发挥止血安胎的作用。Objective： To investigate the clinical efficacy of Baoyin Decoction combined with dydrogesterone tablets in treatment of early threatened abortion. Methods： A total of 60 patients with early threatened abortion were randomly divided into control group and treatment group,with 30 cases in each group. Control group orally took dydrogesterone tablets,while observation group orally took Baoyin decoction based on control group,with one course of treatment in two groups（ 3 weeks）. The clinical main symptom score and total scores,clinical efficacy,serum β-HCG,P,CA125 levels were compared between the two groups. Results： The clinical effective rate of treatment group and control group were 93. 3% and 80%,with statistical difference（ P 〈0. 05）. The improvement of vaginal haemorrhage,lower abdomen pain or distension,low back pain in treatment group was better than that in control group（ P 〈0. 05）. After treatment,the levels of P and β-HCG in two groups increased,and treatment group was higher than control group（ P 〈0. 05）. After treatment,the serum CA125 of two groups decreased,and there was statistical difference between treatment group and control group（ P 〈0. 05）. Conclusion： Baoyin Decoction combined with dydrogesterone tablets in treatment of early threatened abortion can increase clinical efficacy,enhance the levels of serum P and β-HCG,down-regulate the level of serum CA125,decrease the risks of abortion,achieve hemostasis effects and prevent miscarriage.
