部分拔甲、磨钻清除及甲沟重建联合10%高渗盐水治疗嵌甲型甲沟炎的疗效

摘　　要：目的探讨采用部分拔甲+磨钻深度清除甲床、甲根部基质及骨膜+扩大切除甲皱襞甲沟重建术联合10%高渗盐水治疗嵌甲型甲沟炎的临床效果。方法 2015年6月～2017年6月,对74例共86足趾嵌甲型甲沟炎患者采取部分拔甲+磨钻深度清除甲床、甲根部基质及骨膜+扩大切除甲皱襞甲沟重建术联合10%高渗盐水治疗甲沟炎,分析嵌甲型甲沟炎的临床特点和治疗疗效。结果所有患趾的伤口均一期愈合,并实现了甲沟一期成形。术后随访74例患者9～12个月,均无复发,趾甲外形稍缩窄。结论该术式从根源上彻底清除病灶、治愈率高、创伤小,病程短,趾甲外形稍缩窄,值得推广。Objective To explore the clinical effect of partial removal of a nail＋ drill to remove the nail bed, base material of the nail base and periosteum ＋ expanded resection of nail creases and 10% hypertonic saline in the treatment of incarceration of paronychia. Methods From June 2015 to June 2017, a total of 74 patients with 86 toe ingrown nail parotitis took a partial armor ＋ drill deeper to remove the nail bed, nail base matrix and periosteum ＋ expanded resection of nail folds, reconstruction of the nail groove Combined with 10% hypertonic saline, the clinical features and therapeutic efficacy of incarceration of paronychia were analyzed. Results All the wounds of the affected toe were healed in one stage and a first-stage formation of a ditch was achieved. Seventy-four patients were followed up for 9 to 12 months after surgery. There was no recurrence and the toenail was slightly narrowed. Conclusion The surgical method completely removes the lesions from the root, has a high cure rate, minimal trauma, a short course of disease, and a slightly narrowed toenail profile, which is worthy of widespread promotion.
