超声对甲状腺微小乳头状癌合并桥本甲状腺炎诊断效能

摘　　要：目的研究超声对甲状腺微小乳头状癌(PTC)合并桥本甲状腺炎(HT)患者的临床诊断价值。方法 71例PTC患者均经手术病理确诊。单纯PTC组(n=42)和合并HT组(n=29)。所有患者术前均采用超声进行检查,术后与病理情况对比,分析超声对两组患者的诊断效能。结果单纯PTC组淋巴结转移率明显高于合并HT组,P <0. 05;其中以中央区转移多见。两组患者总确诊率、侧颈部转移确诊率均无差异,P> 0. 05;而单纯PTC组中央区转移确诊率明显高于合并HT组,P <0. 05。结论超声对单纯PTC患者淋巴结转移率检测更准确,而对合并HT的PTC患者诊断率较低,但对于侧颈部淋巴结转移的诊断并不受HT影响。Objective To study the clinical diagnostic value of ultrasound in small papillary thyroid carcinoma（ PTC） with Hashimoto＇s thyroiditis（ HT）. Methods 71 cases of patients were diagnosed with PTC by operation and pathology. These patients were divided into simple PTC group（ n = 42） and HT group（ n = 29）. Then,all the patients were examined by ultrasound before operation,and the diagnostic efficacy of ultrasound on the two groups was analyzed by comparing the postoperative and pathological conditions. Results The lymph node metastasis rate of the simple PTC group was significantly higher than that of the HT group（ P〈0. 05）,where most of the cases were central zone metastasis. There was no difference between the two groups in the diagnosis rates of total and lateral neck metastasis（ P〉0. 05）. However,the diagnosis rate of central zone metastasis in simple PTC group was significantly higher than that in HT group（ P〈0. 05）. Conclusion Ultrasonography is more accurate in detecting lymph node metastasis in patients with simple PTC while the diagnosis rate is low in PTC patients with HT,but the diagnosis of lateral neck lymph node metastasis is not affected by HT.
