十二指肠喂养对先天性食道闭锁患儿术后营养状态及代谢的影响

摘　　要：目的探讨十二指肠喂养对先天性食道闭锁(congenital esophageal atresia,CEA)患儿术后营养状态及代谢的影响。方法便利抽样法选择2012年10月至2017年12月广东省中山市博爱医院新生儿科收治并行手术治疗的Ⅲ型CEA新生儿40例为研究对象,将2012年10月至2015年3月收治的15例患儿作为对照组,给予鼻胃管喂养;2015年4月至2017年5月收治的25例患儿为观察组,给予十二指肠喂养,观察比较两组患儿术后第7、14d的血清白蛋白(albumin,ALB)、血清前白蛋白(phantom assassin,PA)水平等营养状态指标和总热卡、蛋白质及蛋白质能量比等代谢情况。结果术后第7、14d,观察组患儿的总热卡、蛋白质及蛋白质能量比均优于对照组,ALB、PA水平均高于对照组,差异均有统计学意义(均P<0.01)。结论十二指肠喂养较鼻胃管喂养更能促进CEA术后患儿的生长代谢,改善患儿的营养状况。Objective To investigate the effects of postoperative duodenal feeding on the nutritional status and metabolism in children with congenital esophageal atresia （CEA）. Methods By convenience sampling method,40 children with CEA surgery were selected in the study. A total of 15 children admitted from Oc tober 2012 to March 2015 were selected in the control group,and received nasogastric tube feeding; 25 ca ses from April 2015 to May 2017 were set as observation group and received duodenal feeding. The level of albumin and phantom assassin,total calories,protein and the ratio of protein and energy were compared be tween two groups at 7 d and 14 d postoperative. Results Patients＇ total calories,protein and the ratio of protein and energy in the observation group were better than those of the control group at 7 d and 14 d postoperative. The albumin and phantom assassin in the observation group were significantly higher than those in the control group, and the difference was statistically significant （P〈0. 01）. Conclusions The du odenal feeding is more effective in promoting the growth and metabolism of children after CEA surgery than nasogastric tube feeding,thus improves the nutritional status of children.
