搔刮术对内膜形态不良患者冷冻周期妊娠结局的影响

摘　　要：目的评估子宫内膜搔刮术对冷冻胚胎移植周期中内膜形态不良患者临床结局的影响。方法选择2015年4月至2016年12月间在第四军医大学西京医院生殖中心进行人工周期冷冻胚胎移植,内膜形态呈C型的患者136例,在月经周期第5~7天行阴道超声检查,评估内膜形态,将其分为两组,A组进行子宫内膜搔刮,B组不做特殊处理,对比两组患者该冷冻移植周期的临床结局。结果 A组患者搔刮术后子宫内膜形态得到改善者占72.62%(61/84),显著高于B组的17.31%(9/52)(χ2=39.340,P<0.05),两组患者转化日内膜厚度比较无显著性差异(t=0.287,P>0.05)。A组的胚胎种植率及临床妊娠率分别为20.43%(38/186)、41.46%(34/82),B组分别为11.29%(14/124)和21.15%(11/52),A组胚胎种植率和临床妊娠率均显著高于B组(χ2值分别为4.452、5.885,均P<0.05)。结论子宫内膜搔刮术能改善子宫内膜形态,提高冷冻胚胎移植周期的临床妊娠率。Objective To evaluate the effects of endometrial scraping on pregnancy outcomes of frozen embryo transfer cycles with abnormal endometrium.Methods All patients underwent transvaginal ultrasound examination on the 5-7 th day of menstrual cycle,and the endometrial morphology was evaluated.One hundred and thirty-six patients with type C endometrium were picked out and divided into group A accepting endometrial scraping and group B not receiving special treatment.The pregnancy outcomes of two groups were compared.Results The endometrial morphology was improved in 72.62%（61/84）of patients in group A,which was significantly higher than that（17.31%,9/52）in group B（χ2=39.340,P〈0.05）.The thickness of endometrium on transformation day between two groups had no significant difference（t=0.287,P〉0.05）.The implantation rate and clinical pregnancy rate of group A was 20.43%（38/186）and 41.46%（34/82）,respectively,and those of group B were 11.29%（14/124）and 21.15%（11/52）,respectively.Both implantation rate and clinical pregnancy rate of group A were significantly higher than those of group B（χ2 value was 4.452 and 5.885,respectively,both P〈0.05）.Conclusion Endometrial scraping in frozen embryo transfer cycles could improve the endometrial morphology and clinical pregnancy rate of frozen embryo transfer cycles.
