摘　　要：目的调查新生儿重症监护病房(NICU)医护人员手卫生依从性,找出薄弱环节。方法采用前瞻性观察研究,选取2017年1至3月在山东大学第二医院在NICU工作的医护人员,随机调取各个时段、各个病室监控视频。应用NICU监控视频观察医护人员手卫生执行情况,统计并分析医护人员手卫生依从性及其影响因素。结果不同工作年限医护人员间手卫生依从性差异存在统计学意义(χ2=6.566,P<0.05)。夜班与白班医护人员手卫生依从性差异存在统计学意义(χ2=5.641,P<0.05)。应用辅助通气患儿的医护人员手卫生依从性高于未进行辅助通气患儿的医护人员,差异存在统计学意义(χ2=11.113,P<0.01),且不同病室间医护人员手卫生依从性差异存在统计学差异(χ2=8.669,P<0.05)。结论 NICU医护人员手卫生依从性整体尚可,低年资医护人员、夜班人员、未进行呼吸机辅助通气患儿的医护人员、普通监护室及早产恢复室医护人员手卫生依从性较低,应有针对性采取相应措施,提高其手卫生依从性。Objective To study the factors associated with the hand hygiene compliance of medical staff in neonatal intensive care unit（NICU）to find out weak link.Methods Perspective study was conducted to investigate the hand hygiene compliance of medical staff in NICU in Second Hospital of Shandong University by monitoring video randomly selected at different time and units.Factors that associated with the hand hygiene compliance of the medical staff in NICU were analyzed.Results There was significant difference in hand hygiene compliance between medical staff with different working years（χ2=6.566,P〈0.05）.The difference was significant in hand hygiene compliance between medical staff on day shift and on night shift（χ2=5.641,P 0.05）.The hand hygiene compliance was higher in medical staff serving mechanical ventilation infants than in those without ventilation task（χ2=11.113,P〈0.01）,and that was statistically different between medical staff working in different wards（χ2=8.669,P〈0.05）.Conclusion Hand hygiene compliance of medical staff in NICU is acceptable.It is low in junior staff,staff on night shift,staff without mechanical ventilation task,and staff working in general ICU and premature recovery room,so corresponding measures should be taken to improve their hand hygiene compliance.
