摘  要:目的了解2型糖尿病患者微量白蛋白尿、肾小球滤过率下降是否平行。方法2013年4月至2015年3月纳入北京市27个社区卫生中心已确诊的2型糖尿病患者1814例为研究对象,进行体格检查及尿白蛋白肌酐比(ACR)、血肌酐等相关实验室检查,应用慢性肾脏病流行病学合作研究公式(CKD-EPI)、Cockcroft-Gauh公式(CG)和简化肾脏病膳食改良公式(MDRD)计算预估肾小球滤过率(eGFR),分别记为eGFR(EPI)、eGFR(CG)、eGFR(MDRD)。两组数据均值比较应用t检验,中位数比较应用Mann-Whitney检验,率的比较应用卡方检验,相关性检验应用Spearman相关分析。结果纳入男性754人(41.6%),女性1060人(58.4%),平均年龄(61±9)岁。ACR与eGFR(EPI)(r=-0.013)、eGFR(CG)(r=-O.014)、eGFRM(MDRD)(r=-0.007)无相关性(均P〉0.05)。2型糖尿病患者微量白蛋白尿与eGFR下降不平行,83.3%(15/18)的大量白蛋白尿的男性患者和50%(11/22)的大量白蛋白尿的女性患者eGFR正常;21%(101/482)的男性患者和30.7%(238/776)的女性患者尿白蛋白尚正常而eGFR〈90ml·min^-1。·(1.73m^2)^-1,2.5%(12/482)的男性患者和3.4%(26/776)的女性患者尿白蛋白尚正常而eGFR〈60ml·min·(1.73m^2)^-1。结论2型糖尿病患者尿微量白蛋白不能完全反映’肾功能受损情况。Objective To explore whether changes of microalbuminuria are paralleled with the reduced glomerular filtration rate in type 2 diabetic patients. Methods A study was conducted in 1 184 type 2 diabetic patients from 27 Beijing community clinics between Apr. 2013 and Mar. 2015. Data of albumin/creatinine ratio (ACR), serum creatinine and lipid profiles were collected. Estimated glomerular filtration rate (eGFR) was evaluated by Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI), Cockcroft-Gault(CG), simplified Modification of Diet in Renal Disease (MDRD) formulas, and designated as eGFR(EPI), eGFRcG, eGFR(MDRO), respectively. All data were expressed as^-x:s or median (interquartile range) as appropriate. The Student's t test or Mann-Whitney test was used for between-group comparison for continuous variables, and the Chi-square test for categorical variables. Spearman Rank Correlation was used to find the correlation between ACR and eGFR. Results Of all type 2 diabetic patients aged (61±9) yrs, 754 were males (41.6%) and 1 060 were females (58.4%). ACR was not correlated with eGFRE(EPI) (r=-0.013), eGFR(CG) (r=-0.014) or eGFR(MDRD) (r=-0.007). The changes of microalbuminuria were not paralleled with the reduced glomerular filtration rate. Of patients with macroalbuminuria (defined as ACRe〉300 mg/g), 83.3% (15/18) male patients and 50% (11/22) female patients had eGFR(EPI)〉90 ml. min^-1 (1.73 m^2)^-l. Of patients with normal albuminuria (defined as ACR〈22.1 mg/ g in male and ACR〈30.9 mg/g in female), 21% (101/482) male patients and 30.7% (238/776) female patients had eGFR(EPI)〈90 ml·min^-1 (1.73 m^2)^-1, and 2.5% (12/482) male patients and 3.4% (26/776) female patients had eGFR(EPI)〈60 ml·min^-1 (1.73 m^2)^-1. Conclusion Mieroalbuminuria can not fully reflect the reduced glomerular filtration rate in type 2 diabetic patients.
