早产儿初乳口腔免疫疗法联合母乳喂养对体质量增长的影响

摘　　要：目的分析早产儿初乳口腔免疫疗法(OIT)联合母乳喂养对体质量增长的影响。方法 86例早产儿随机分为对照组和观察组,各43例。对照组实施母乳喂养,观察组实施初乳口腔免疫疗法(OIT)联合母乳喂养。分析2组肠外营养时间和体质量增长情况。结果观察组开始肠内喂养时间、奶量达到120 mL/(kg·d)的时间、肠外营养时间均明显短于对照组,差异有统计学意义(P <0. 01)。观察组体质量增长速率、头围增长速率均高于对照组,差异有统计学意义(P <0. 01)。观察组喂养不耐受发生率20. 93%,低于对照组的41. 86%,差异有统计学意义(P <0. 05)。观察组早产儿再入院率低于对照组(P <0. 05)。结论 OIT联合母乳喂养可缩短早产儿肠外营养时间,尽早实现肠内喂养,降低喂养不耐受风险,且可以促进早产儿体质量增长,降低早产儿的生长发育迟缓率和再入院率。Objective To investigate the effect of oral immunotherapy （OIT） combined with breastfeeding on weight gain in preterm infants. Methods Totally 86 premature infants were randomly divided into the control group and the observation group, with 43 cases in each group. The control group received breastfeeding, and the observation group received colostrum OIT combined with breastfeeding. The duration of parenteral nutrition and weight gain of infants were observed and compared between two groups. Results The time to start enteral feeding, time to milk yield reaching 120 mL/（kg @ d） and duration of parenteral nutrition in the observation group were shorter than those in the control group （P 〈 0.01 ）. Infants in the observation group had a higher growth rate of body weight and head circumference compared with those in the control group（P 〈0.01 ）. The incidence of feeding intolerance was lower in the observation group than that of controls （20.93% vs. 41.86%, P 〈0.05）. The readmission rate of infants was lower in the observation group than that of the control group （ P 〈 0.05 ）. Conclusion The OIT combined with breastfeeding can effectively shorten the duration of parenteral nutrition and promote the weight gain in premature infants, which in turn reduces the growth retardation rate and readmission rate of premature infants.
