桃红四物汤对髋关节置换术患者髋关节功能的影响

摘　　要：目的观察桃红四物汤对髋关节置换术患者髋关节功能的影响。方法选取86例行择期人工髋关节置换术患者,按随机数字表法分为观察组与对照组各43例。2组患者均行持续硬膜外麻醉下全髋关节置换术,对照组术后予低分子肝素,观察组在对照组基础上予桃红四物汤,每日1剂,每日2次,口服。评价2组术后1个月临床疗效,观察术前3 d及术后1个月全血高切黏度、全血低切黏度、血浆比黏度,采用Harris评分量表评估患者术后1个月及随访1、2年的髋关节功能。结果观察组总有效率为86.05%(37/43),对照组为81.40%(35/43),2组比较差异无统计学意义(P>0.05)。与本组术前3 d比较,2组术后1个月全血高切黏度、全血低切黏度、血浆比黏度显著下降(P<0.05);观察组术后1个月全血低切黏度、血浆比黏度显著低于对照组(P<0.05)。与本组术后1个月比较,2组随访1、2年的Harris各单项评分及总分均显著升高(P<0.05);观察组术后1个月及随访1、2年的运动范围评分、关节功能评分及总分均显著高于对照组(P<0.05)。结论桃红四物汤可促进髋关节置换术患者术后的血液循环,强化患者早期功能锻炼效果,改善髋关节功能。Objective To observe the effects of Taohong Siwu Decoction on hip functions of patients with hip arthroplasty. Methods Totally 86 patients with hip arthroplasty were selected and divided into observation group and control group through random number table method, with 43 cases in each group. Both groups received total hip arthroplasty with continuous epidural anesthesia. The control group was treated with low molecular weight heparin, and the observation group was treated with Taohong Siwu Decoction on the basis of the control group, one dosage per day, twice a day, orally. The clinical efficacy after 1 month of surgery of both groups were evaluated. Whole blood high shear viscosity, whole blood low shear viscosity, and plasma specific viscosity at 3 d before surgery and 1 month after surgery were observed. The Harris score scale was used to assess hip functions in patients 1 month after surgery and after 1 and 2 years of surgery were followed up. Results The total effective rate was 86.05%（37/43） in the observation group and 81.40%（35/43） in the control group, without statistical significance（P〈0.05）. Compared with 3 d before surgery, whole blood high shear viscosity, whole blood low shear viscosity, and plasma specific viscosity at 1 month after surgery in both groups decreased significantly（P〈0.05）; whole blood low shear viscosity and plasma specific viscosity in the observation group at 1 month after surgery were obviously lower than the control group（P〈0.05）. Compared with 1 month after surgery, Harris scores in the follow-up of both groups after 1 and 2 years of surgery increased significantly（P〈0.05）; exercise range scores, hip function scores and total scores of 1 month after surgery and 1 and 2 years follow-up after surgery in the onservation group were significantly higher than those in the control group（P〈0.05）.
