慢性病保健扩展模型在社区高血压干预中的应用效果分析

摘　　要：目的通过评价慢性病保健扩展模型在社区高血压干预的实践效果,探索适宜的社区高血压防治模式。方法采用随机抽样及实验对照研究法,从武汉市35个公立社区卫生服务中心中随机抽取600名高血压患者,分为干预组和对照组,干预组采用慢性病保健扩展模型进行干预,对照组采取常规干预,6个月后比较两组的血压值及生命质量变化情况。结果干预前后,两组的收缩压差值差异有统计学意义(P<0.05),舒张压差值无显著差异;在生命质量方面,干预组在躯体疼痛、社会功能、心理健康总分、生理健康总分4个维度好转程度要高于对照组(P<0.05)。结论在社区高血压管理中,应用慢性病保健扩展模型可以提高患者血压控制效果及患者的生命质量。Objective This study intends to explore appropriate intervention model for hypertension in community by evaluation of the application of expanded chronic disease care model in community practice.Methods 600 hypertension patients were selected randomly from 35 public community health care centers in Wuhan by random stratified sampling and experiment comparison.Then they were divided into intervention and control group,with the expanded model being used in the intervention group and standard intervention methods for the control group.Data about changes in blood pressure and the quality of life were compared between two groups after 6 months.Results The difference between two groups of changes in the systolic blood pressure was statistically significant（P〈0.05）,but no significant difference was observed for diastolic blood pressure.In terms of quality of life,intervention group scored higher than control group in 4 dimensions：pain in the body,social functioning,mental health and physical health.Conclusion The effectiveness of blood pressure control and quality of life of hypertension patients can be greatly improved by applying the expanded chronic disease care model for the hypertension management in community.
