双腹腔镜完全腹膜外疝修补术与完全腹膜外疝修补术疗效观察

摘　　要：目的:比较双腹腔镜TEP与单纯TEP治疗成人腹股沟疝的临床疗效。方法:回顾性分析安徽医科大学第三附属医院2016年4月至2017年9月收治的137例成人腹股沟疝患者临床资料,根据手术方式分成双镜TEP组和TEP组,其中应用双腹腔镜完全腹膜外腹股沟疝修补术为双镜TEP组共71例,完全腹膜外腹股沟疝修补术为TEP组共66例,观察两组患者的手术时间、术后住院时间、术中探查对侧隐匿性疝、血肿或血清肿、疼痛及感觉异常、并发症的发生率。结果:137例患者手术均顺利完成,手术时间双镜TEP组(57. 6±8. 5) min,TEP组(53. 2±7. 4) min,两组患者手术时间差异有统计学意义(P <0. 05),在探查对侧隐匿疝上比较,差异有统计学意义(P <0. 05),术后住院天数及并发症比较,差异无统计学意义(P> 0. 05),随访无1例复发。结论:双腹腔镜TEP和TEP治疗腹股沟疝均是安全、有效的。Objective： To compare the clinical effect between totally extraperitoneal prosthesis（ TEP） with double laparoscopes and TEP alone in the treatment of inguinal hernia. Methods： A total of 137 cases with inguinal hernia from April 2016 to September 2017 in No. 3 Hospital Affiliated to Anhui Medical University were enrolled. The patients were divided into 2 groups： 71 patients treated by TEP with double laparoscopes as group A and 66 patients with TEP as group B. The operation time,postoperative hospitalization,intraoperative peering into the contralateral hidden hernia,hematoncus and seroma,postoperative pain and paraesthesia,and recurrence rate were compared between the two groups. Results： 137 cases were operated successfully. The operation time was（ 57. 6 ± 8. 5） minutes in group A and（ 53. 2 ± 7. 4） minutes in group B,and the difference was significant（ P 〈 0. 05）. There was significant difference in the detection of contralateral hidden hernia in the two groups（ P 〈 0. 05）.There was no difference in the incidence of postoperative complications and postoperative hospitalization days（ P 〉 0. 05）. The postoperative recurrence was not found in the follow-up. Conclusions： Both TEP with double laparoscopes and TEP are safe and effective in the treatment of inguinal hernia.
