先俯卧位后仰卧位抚触结合穴位按摩对生理性黄疸新生儿依从性及疗效的影响

摘　　要：目的探讨先俯卧位后仰卧位抚触结合穴位按摩对生理性黄疸新生儿依从性及疗效的影响。方法选取我院2017年6月至2018年3月产科接诊的160例新生儿为研究对象,随机将其等分为对照组和试验组,对照组实施先俯卧位后仰卧位抚触干预,试验组在对照组基础上结合穴位按摩。比较两组新生儿治疗依从性、血清总胆红素(TBIL)、临床疗效、新生儿胎便转黄时间及黄疸消退时间。结果试验组新生儿治疗依从性显著高于对照组(P <0. 05);试验组新生儿第1,2天TBIL水平与对照组相比差异无统计学意义(P> 0. 05),第3,4天TBIL水平与对照组相比差异具有统计学意义(P <0. 05),且试验组新生儿第3,4天TBIL水平显著得到有效控制;试验组新生儿胎便转黄及黄疸完全消退时间短于对照组(P <0. 05);试验组新生儿治疗总有效率显著高于对照组(P <0. 05)。结论给予新生儿先俯卧位后仰卧位抚触结合穴位按摩,可有效提高新生儿治疗依从性,改善胆红素水平,缩短新生儿生理性黄疸时间,值得临床推广应用。Objective To explore the effect of supine position after prone position caress combined with acupoint massage on compliance and cure of neonates with physiologic jaundice. Methods To compared the therapeutic compliance, serum total bilirubin （TBIL）, clinical efficacy, the time of meconium turning into yellow, and the time of jaundice regression, 160 cases of neonates treated in our hospital from June 2017 to March 2018 were observed, and were randomly divided into control group and study group with equal members. The control group adopted the supine position after prone position caress intervention, while the study group adopted the acupoint massage on the basis of the control group. Results The therapeutic compliance of newborns in the study group was significantly higher than that in the control group （ P 〈 0.05 ）. There was no statistical significance in the difference of TBIL levels of the first day and the second day between the study group and the control group（ P 〉 0.05 ） , the difference of TBIL levels of the 3rd and 4th day were statistically mattered between the two groups （P 〈 0.05 ）, with which the TBIL levels of the third and fourth day in the study group were effectively controlled. The time of meconium turning into yellow and the time of jaundice regression in the study group were shorter than that in the control group （ P 〈 0.05 ）. The total efficiency of the cure in the study group was significantly higher than that in the control group（P 〈 0.05 ）. Conclusion The application of supine position after prone position caress combined with acupoint massage on neonates could effectively improve the treatment compliance and TBIL level, and shorten the time of physiologic jaundice. It was worthy of clinical promotion and application.
