摘 要:目的探讨北京部分地区女性人乳头瘤病毒(HPV)感染情况与基因型分布特征,为女性宫颈癌和尖锐湿疣的预防和治疗提供依据。方法收集2016年12月至2017年11月间就诊于首都医科大学附属北京佑安医院妇科和皮肤性病科的2 116例女性宫颈脱落细胞标本,利用高危/低危型HPV分型核酸测定试剂盒(荧光PCR法)进行基因分型检测,并分析其分布特征。结果 2 116例标本中检测出HPV阳性者507例,感染率为23.96%,HPV高危阳性标本中,感染率最高的前3位是HPV-16(21.89%)、HPV-52(20.91%)、HPV-58(15.58%),不同年龄组HPV检出率比较差异具有显著统计学意义(P<0.01),其中在≤20岁和≥61岁两个年龄段中,HPV检出率最高,分别为53.49%和40.63%。不同疾病状态下,宫颈上皮内瘤变组阳性率最高的3种基因型分别为HPV-16(20.94%)、HPV-52(18.41%)、HPV-58(12.64%),尖锐湿疣组阳性率最高的3种型别分别为HPV-6&HPV-11(43.65%)、HPV-16(12.70%)、HPV-52(12.70%),低危合并高危HPV感染率为22.2%。结论北京部分地区女性最常见的HPV感染基因型为HPV-16、HPV-52和HPV-58,≤20岁和≥61岁年龄组为HPV感染高发人群,HPV基因分型检测可以对宫颈癌、尖锐湿疣的预防和治疗提供可靠的临床依据。Objective To evaluate the human papilloma virus(HPV) infection status and genotype distribution features of women in parts of Beijing, in order to provide basis for prevention and treatment of cervical cancer and condyloma acuminatum. Methods Cervical exfoliated cell specimens of 2 116 females were collected, who were treated in Department of Gynaecology, Department of Dermatology STD, Beijing You'an Hospital Affiliated to Capital Medical University between Dec. 2016 and Nov. 2017. The high/low-risk type HPV nucleic acid determination kit(fluorescence PCR method) was used for genotyping. The distribution characteristics were analyzed. Results Among the 2 116 cases,507(23.96%) were found HPV positive. In the positive cases, the top three genotypes were HPV-16(21.89%), HPV-52(20.91%) and HPV-58(15.58%). The infection rate of HPV showed significant difference among different age groups(P〈0.01). The infection rate was the highest in ≤20 years group(53.49%), followed by ≥61 years group(40.63%). In cervical intraepithelial neoplasia group, the top three genotypes detected were HPV-16(20.94%), HPV-52(18.41%) and HPV-58(12.64%). In condyloma acuminatum group, the top three genotypes detected were HPV-6 HPV-11(43.65%), HPV-16(12.70%) and HPV-52(12.70%), and co-infection rate of low-and high-risk type HPV was 22.2%.Conclusion The most common HPV subtypes are HPV-16, HPV-52 and HPV-58 in parts of Beijing, and HPV infection are mostly occurred in people ≤20 years old and ≥61 years old. HPV genotyping can provide reliable clinical evidence for the prevention and treatment of cervical cancer and condyloma acuminatum.
