摘  要:目的:观察不同时间点针刺脑出血(ICH)模型大鼠内关、水沟穴对血脑屏障基质金属蛋白酶-2/9基因(MMP-2/9)和血管内皮生长因子(VEGF)表达的影响,探讨针刺对血管新生及神经保护的作用机制。方法:将135只雄性Wistar大鼠根据随机数字表法分为假手术组、造模3 h组、造模9 h组、造模24 h组、造模48 h组、3 h针刺组、9 h针刺组、24 h针刺组、48 h针刺组,每组各15只;采用尾状核自体血缓慢注入法制作ICH模型,并采用免疫组化法检测MMP-2/9及VEGF阳性细胞计数。结果 :3、9、24、48 h针刺组大鼠血脑屏障MMP-2、MMP-9阳性细胞较同一时间点造模组均明显减少;3 h针刺组与9、24、48 h针刺组比较,MMP-2、MMP-9阳性细胞减少程度更加明显;3、9 h针刺组大鼠血脑屏障VEGF阳性细胞较同一时间点造模组明显减少,但在24、48 h针刺组VEGF阳性细胞较造模组明显增多,差异具有统计学意义(P<0.05)。结论:不同时间点针刺内关、水沟穴可抑制脑出血大鼠MMP-2/9阳性细胞的表达,增加VEGF阳性细胞的表达;脑出血发生后针刺时间越早越能促进血管新生及神经再生,达到保护脑神经系统的作用。Objective: To observe the effect of acupuncture at different time points on the expression of matrix metalloproteinase-2/9(MMP-2/9) and vascular endothelial growth factor(VEGF) in rats at acupoints Neiguan and Shuigou, so as to explore the mechanism of acupuncture on angiogenesis and neural protection. Methods: A total of 135 male Wistar rats were randomly divided into mock surgical group, model 3 h group, model 9 h group, model 24 h group, model 48 h group, 3 h acupuncture group, 9 h acupuncture group,24 h acupuncture group, 48 h acupuncture group, with 15 animals in each group; the ICH model was made by the caudate nucleus autologous slow blood injection method and the MMP-2/9 and VEGF positive cell counts were detected by immunohistochemistry. Results: The blood-brain barrier MMP-2, MMP-9 and VEGF positive cells in the 3, 9, 24, 48 h acupuncture groups were significantly lower than those of model group at the same time, compared with the 9, 24, 48 h acupuncture groups, the reduction of MMP-2 and MMP-9 positive cells was more obvious in the 3 h acupuncture group. The VEGF-positive cells in the blood-brain barrier of 3, 9 h acupuncture groups were significantly lower than the model group at the same time point, but, VEGF-positive cells of 24, 48 h acupuncture groups was significantly more than the model group at the same time point, with statistically significant differences(P0.05). Conclusion: Acupuncture Neiguan and Shuigou at different time points can inhibit the expression of MMP-2/9 positive cells of rats with intracerebral hemorrhage and increase the expression of VEGF positive cells, and the earlier the acupuncture time after cerebral hemorrhage, the more can it promote angiogenesis and nerve regeneration, to protect the brain's nervous system.
