接纳与承诺疗法在经皮射频消融术患者中的应用

摘　　要：目的探讨接纳与承诺疗法对行经皮射频消融术患者心理弹性与生活质量的影响。方法将90例行经皮射频消融术的甲状腺癌患者随机分为观察组和对照组各45例。对照组患者接受常规护理,观察组在此基础上辅以接纳与承诺疗法进行护理干预,并于干预前后采用心理弹性量表评分(CD-RISC)、生命质量测定量表评分(QLQ-C30)以及护理满意度指标进行评价。结果干预1月后,观察组心理弹性中自强、坚韧、乐观维度评分显著高于对照组;生命质量中躯体功能、社会功能、角色功能、情绪功能、认知功能维度评分显著高于对照组;对环境、爱护与关心、技术与护理服务的满意度评分显著高于对照组(P<0.05,P<0.01)。结论针对行经皮射频消融术的甲状腺癌患者采取接纳与承诺疗法进行护理干预,能够有效改善患者的心理弹性,提高其生活质量和护理服务满意度。Objective To explore the influence of acceptance and commitment therapy on quality of life and resilience of patients undergoing percutaneous radiofrequency ablation for thyroid cancer.Methods A total of 90 patients undergoing percutaneous radiofrequency ablation for thyroid cancer were randomly and evenly divided into a control group and an observation group.The control group received conventional nursing care,while the observation group additionally received acceptance and commitment therapy.The Connor-Davidson Resilience Scale,the EORTC Core Quality of Life Questionnaire（QLQ-C30,version 3.0）and a nursing care satisfaction questionaire were used to evaluate the effect before and after intervention.Results After one month of intervention,the scores of self-improvement,tenacity and optimism of CD-RISC in the observation group were significantly higher than those in the control group;such quality of life dimensions as body function,social function,role function,emotional function,cognitive function were also significantly higher than the controls;the observation group had higher satisfaction scores of environment,caring,nursing skill levels and nursing service than the control group（P〈0.05,P〈0.01）.Conclusion Implementation of acceptance and commitment therapy can effectively enhance resilience,quality of life and satisfaction of patients undergoing percutaneous radiofrequency ablation for thyroid cancer.
