中风胶囊降低脑卒中复发的前瞻性队列研究

摘　　要：目的分析评价中成药中风胶囊在降低脑卒中复发率方面所起到的的作用。方法采用前瞻性队列研究,入选服用中风胶囊的脑卒中病人616人,均接受常规的内科治疗措施,利用受试者脱离服药自然形成的队列差,以服药总月数24个月为标准分为强暴露组295人和弱暴露组321人,入组后收集个人资料、生活习惯、既往病史、伴随服药、血压情况以及脑卒中危险因素、伴发临床疾患等资料,连续追踪调查36个月,分别记录其终点事件(脑梗死、脑出血、死亡等)的发生率。结果弱暴露组的终点事件发生率为13. 08%(42/321);强暴露组的终点事件发生率为7. 80%(23/295)。两组比较有显著性差异(χ~2=4. 55; P=0. 035; OR:0. 56; 95%CI:0. 33-0. 96)。在进行组间的均衡性检验中,有"饮食是否规律"、"运动情况"、"是否并发高血压及糖尿病""是否合并使用阿司匹林""丹红注射液""脑心通胶囊"存在组间不均衡现象。结论长期服用中风胶囊能明显降低再次发生脑卒中的发病率,服用超过24个月预防效果更好。Objective To explore the effect of ZhongFeng Capsule for the reducing the recurrence rate of stroke. Methods We used prospective cohort study design,616 cases of cerehral infarction treated with ZhongFeng Capsule were included. All patients received standard secondary preventive measures for stroke. According to whether they were taking the ZhongFeng Capsule for two years or short one, the patients have been divided to two groups, the exposed group （295 cases） and weak exposed group （321 cases）, followed - up investigation periodically for 6 months, we collected personal data, life habits, past medical history, concomitant medication, blood pressure, risk factors of stroke and clinical diseases after admission. Their outcome events will be recorded （ cerebral infarction, cerebral hemorrhage, death）. Results The rate of outcome events of the exposed group was 7.8%, while the rate of weak exposed group was 13.08% （ X2 = 4.55 ;P = 0. 035 ; OR： 0.56; 95% CI： 0.33 -0. 96）. There are significant differences between groups. In the balance test between groups, regular diet , exercise , hypertension, diabetes mellitus, aspirin, Danhong injection and Naoxintong Capsule are disproportionate in groups. Conclusion Long - time use of ZhongFeng Capsule could significantly decrease the rate of the recurrence of cerebral infarction. Take more than 24 months, better preventive effect.
