摘  要:目的探讨"三通针法"电针治疗脊髓损伤大鼠的时间窗效应及作用机制。方法 144只Sprague-Dawley雌性大鼠随机分成假手术组(A,n=48)和造模组(n=96)。造模组大鼠采用改良Allen重物打击法制作脊髓损伤大鼠模型,随机分为模型组(B,n=48)、电针组(C,n=48)。电针治疗取大椎、腰阳关及双侧次髎、足三里,选择疏密波,持续时间20min,每天1次。分别在造模后第1、8、15天介入电针治疗,治疗前进行大鼠Basso-Beattie-Bresnahan (BBB)评分,每一个介入治疗点的大鼠随机再分成2个亚组,分别持续治疗7d和14d,治疗结束后提取损伤处脊髓组织; B组分别于造模后第1、3、7、14、21、28d及A组在相应时间点处死大鼠提取脊髓组织,处死大鼠前进行BBB评分。采用专用试剂盒检测脊髓组织胞浆型磷脂酶A2(cytosolic phospholipase A2,c PLA2)活性、酶联免疫法检测脊髓组织前列腺素E (prostaglandin E,PGE)的含量。结果 B组大鼠术后14d BBB评分才开始有一定的提高,直至28d评分较术后7d才有明显的提高(P <0. 05);经电针治疗,术后1d连续治疗7d,术后7d、14d介入治疗7d和14d,大鼠BBB评分较相应时间点的B组都有明显的提高(P <0. 05或P <0. 01);术后7d和14d介入治疗的评分要高于术后1d介入的评分(P <0. 05或P <0. 01),不同时间点介入电针治疗7d与治疗14d评分比较无明显差异(P> 0. 05)。不同时间点,B组大鼠c PLA2酶活性和PGE含量都明显高于A组(P <0. 01); B组大鼠c PLA2酶活性在造模后呈逐渐升高趋势,PGE也呈现相同的趋势,它们之间存在明显的相关性;经电针治疗后,c PLA2酶活性和PGE含量较B组都有明显的降低(P <0. 05或P <0. 01),术后7d和14d开始介入治疗要低于术后1d治疗(P <0. 05或P <0. 01);术后不同时间点介入电针治疗7d与治疗14d c PLA2酶活性和PGE含量之间比较无明显差异(P> 0. 05)。结论 "三通针法"电针能改善脊髓损伤大鼠后肢运动功能,可能与电针抑制脊髓损伤�Objective To investigate the time window effect of Santong electroacupuncture (EA) on motor function of Rats with Spinal Cord Injury and its mechanism. Methods A total of 144 female Sprague - Dawley rats were randomly assigned to sham op- eration group (group A, n = 48) and model group (n = 96). In the model group, Alleng method was used to make SCI rats mod- el, in which model rats were further subdivided into model control group ( group B, n = 48 ) and EA group ( group C, n = 48 ). The treatment groups were electroacupunctured on Dazhui (GV14) and Yaoyangguan (GV3), bilateral Ciliao (BL32) and Zusanli(ST36) with loosetight wave, for 20 minutes every day. The treatment was begin to performance at thelst, 8th and 15th day after modeling, and the hind limb motor function was assessed with Basso - Beattie - Bresnahan (BBB) score before treatment. The rats at every interventional therapy time were randomly subdivided into two subgroup ,with 7 days of treatment in subgroup, and 14days of treatment in other subgroup. After treatment, injured spinal cord tissue was extracted for detecting. In group B, rats were killed at the 1st, 3rd ,7th ,14th and 28th day after modeling, and injured spinal cord tissue was extracted for detecting at the corresponding time in group A. The hind limb motor function was assessed with BBB score before the rats were killed. Activity of cytosolic phospholipase A2 (cPLA2) in injured spinal cord was detected with special assay kit, and level of prostaglandin E (PGE) with enzyme linked immunosorbent assay kit. Results BBB score of rats in group B was began to increased after 14 days of modeling, and significantly increased until 28 days after modeling( P 〈 0.05 ), compared with the 7th day after modeling. The BBB score of rats was improved after 7days and 14days treatment at the 1st, 8th 15th day after modeling, compared with that at the corresponding time in group B(P 〈0.05 or P 〈0.01 ).
