摘  要:目的:观察头穴针刺对局灶性脑缺血大鼠海马旁回核因子(NF)-κB p 65mRNA、IκB mRNA及白介素(IL)-1β、肿瘤坏死因子(TNF)-α表达的影响,探讨头穴针刺调控缺血性脑血管病炎性反应的机制。方法:SD大鼠随机分为正常组、模型组、药物组及头针组,每组16只。采用线栓法建立大鼠急性局灶性脑缺血模型。药物组腹腔注射吡咯烷二硫代氨基甲酸铵盐(100mg·kg^(-1)·d^(-1)),1次/d,共7d。头针组取双侧顶颞前斜线和顶颞后斜线行快速捻转透刺治疗,每次30min,1次/d,共7d。干预前后进行神经功能缺损评分及神经行为学评分。RT-PCR法检测海马旁回NF-κB p 65mRNA、IκB mRNA的表达,免疫组织化学法检测海马旁回IL-1β、TNF-α的表达。结果:与正常组比较,模型组大鼠神经功能缺损评分和神经行为学评分明显增高(P<0.01),海马旁回NF-κB p 65 mRNA、IL-1β、TNF-α的表达均明显增高(P<0.01),IκB mRNA表达显著降低(P<0.01)。与模型组比较,头针组和药物组大鼠神经功能缺损评分和神经行为学评分明显降低(P<0.01),海马旁回NF-κB p 65 mRNA、IL-1β、TNF-α表达均明显降低(P<0.05),IκB mRNA表达明显增高(P<0.05)。结论:上调缺血后脑组织IκB的表达,抑制NF-κB的解离,进而降低炎性因子IL-1β、TNF-α的表达是头穴针刺控制缺血性脑血管病炎性反应的分子机制之一。Objective To observe the effect of scalp-acupuncture intervention on the expression of parahippocampal factor-κB p 65 mRNA (NF-κB p 65 mRNA), IκB mRNA, interleukin-1 β(IL-1β) and tumor necrosis factor-α(TNF-α) in rats with cerebral ischemia (CI), so as to investigate its molecular mechanisms underlying improving CI by reducing inflammatory response. Methods A total of 64 SD rats in each group. The foca rats were randomized into normal control, model, medication and scalp-acupuncture groups, with 16 CI model was established by middle cerebral artery occlusion (MCAO). Intraperitoneal injection of Pyrrolidine Dithiocarbamate (100 mg o kg-1 o d-1) was administrated for rats in the medication group, once a day for 7 days. For rats of the scalp-acupuncture group, the acupuncture needles were rapidly inserted into bilateral Dingnieqianxiexian (MS 6) and Dingniehouxiexian (MS 7), followed by twirling the needles at 200 cycles/min for 1 min, once again every 10 rain during 30 min's needle retention. The treatment was conducted once a day for 7 days. The neurologic deficit score (0- 5 points, impaired con- sciousness, death, etc. ) and neurological function score (motor, sensory and sensory tests, 0- 10 points) were assessed according to Longa's (1989) and Sch^bitz's (2004) methods, respectively. The expression levels of NF-κB p 65 mRNA and IκB mRNA in the parahippocampus gyrus tissue were detected by fluorescence quantitative reverse transcription-POR, and IL-1 β and TNF-α proteins in the parahippocampus gyrus tissue were detected by immunohistochemistry. Results After modeling, the neu- rologic deficit and neurological function scores and the expression levels of NF-κB p 65 mRNA, IL-1 β and TNF-α in the parahip- pocampus were significantly increased in the model group than in the normal group (P〈0.01), while the expression of IκB mRNA was considerably down-regulated ( P〈 0.01 ).
