胃袖状切除术联合空肠旁路术对2型糖尿病大鼠血糖及胰高血糖素样肽1表达的影响

摘　　要：目的研究胃袖状切除术联合空肠旁路术(sleeve gastrectomy plus jejunal bypss,SG+JJB)对2型糖尿病(type 2diabetes mellitus,T2DM)大鼠血糖及胰高血糖素样肽1(glucagon-like peptide 1,GLP-1)的影响,探讨SG+JJB对T2DM大鼠的降糖效果及机制。方法将24只T2DM大鼠按随机数字表法分为两组,各12只,分别为SG+JJB组、假手术组。测定两组T2DM大鼠术前,术后2周、1个月、2个月、3个月的体质量、空腹血糖(fasting blood glucose,FBG)、空腹胰岛素(fasting insulin,FINS)、糖化血红蛋白(glycated hemoglobin,HbA1c)、GLP-1水平的变化。结果与术前比较,SG+JJB组在术后2周、1个月、2个月、3个月体质量均较术前明显下降(P均<0.01),术后2周FBG亦较术前显著下降,并于术后3个月趋近正常水平(P均<0.01),术后3个月HbA1c较术前显著降低(P<0.01)、FINS较术前显著升高(P<0.01)。与术前比较,SG+JJB组术后2周、1个月、2个月、3个月的空腹GLP-1水平均显著升高(P均<0.01)。相同条件下,假手术组T2DM大鼠术后上述各项指标均无统计学意义(P>0.05)。结论 SG+JJB通过促进大鼠GLP-1释放,增加胰岛素分泌,对降低血糖起着关键作用。Objective To study the effect of sleeve gastrectomy plus jejunal bypss（SG＋JJB）on blood glucose and glucagon-like peptide 1（GLP-1）expression of type 2 diabetes mellitus（T2 DM）rats.And to investigate the hypoglycemic feasibility and possible mechanisms of SG＋JJB in T2 DM rats.Methods Twenty-four T2 DM rats were randomly divided into two groups：SG＋JJB group（n=12）and sham operation group（n=12）.The body weight,fasting blood glucose（FBG）,fasting insulin（FINS）,glycated hemoglobin（HbA1 c）and GLP-1 levels of T2 DM rats in two groups were determined before operation,2 weeks,1 month,2 months,3 months after operation.Results The body weight and FBG levels of T2 DM rats in SG＋JJB group were significantly decreased at 2 weeks,1 month,2 months,3 months after operation than those before operation（all P〈0.01）.Meanwhile,compared with pre-operation,HbA1 clevels of T2 DM rats in SG＋JJB group were decreased（P〈0.01）and FINS levels of T2 DM rats in SG＋JJB group were increased at 3 months after operation（P〈0.01）.GLP-1 expressions were significantly increased at 2 weeks,1 month,2 months,3 months after operation than those before operation（all P〈0.01）.Under the same conditions,the above parameters did not change in sham operation group after operation（P〉0.05）.Conclusion SG＋JJB has obvious hypoglycemic effect on T2 DM rates.SG＋JJB can make the food quickly pass through ileum and stimulate the GLP-1 secretion,increase insulin release.
